Resume Objectif : Comprendre la physiologie normale de la thermoregulation, la pathophysiologie et les signes et symptomes associes a l’epuisement du a la chaleur et au coup de chaleur. Comprendre les particularites de la personne âgee qui la predisposent a des troubles relies a la chaleur. Reconnaitre les medicaments qui peuvent aggraver ou precipiter des troubles relies a la chaleur. Sources des donnees : Une revue de la litterature a ete effectuee par la consultation de references primaires a partir de PubMed avec les mots cles suivants : epuisement a la chaleur, coup de chaleur, personnes âgees, troubles de la thermoregulation et medicaments. Des references secondaires et tertiaires ont egalement ete consultees. Analyse des donnees : Plusieurs etudes epidemiologiques font etat d’une surmortalite chez les personnes âgees durant les vagues de chaleur mais ne rapportent que rarement la prise de medicaments par les victimes. Par contre, il existe dans la litterature plusieurs cas de troubles relies a la chaleur chez des patients prenant des medicaments specifiques. Certains medicaments peuvent mettre la personne âgee a risque de precipiter ou d’aggraver un trouble relie a la chaleur. Conclusion : Il faut faire preuve de prudence dans l’utilisation des medicaments chez les personnes âgees durant les periodes chaudes de l’ete. Des mesures de prevention tels la diminution des doses, le changement ou le retrait de certains medicaments peuvent etre envisagees apres une analyse cas par cas. Des mesures de prevention sont proposees durant les vagues de chaleur. Abstract Objective : To identify age-related physiological changes that may predispose the elderly to thermoregulatory disorders (hyperthermia, dehydration, heat exhaustion and heatstroke). Identify the medication which can induce thermoregulatory disorders. Data extraction : A literature review was conducted using the PubMeb database to find primary references. Secondary and tertiary references were also used. Data Analysis : Various epidemiological studies have established an increased mortality rate in the elderly during heat waves, but they rarely report the patient’s medications. However, there are numerous documented cases of heatstroke or thermoregulatory disorders in patients taking specific medications. Few serious articles mention the inherent risks of medicating geriatric patients during heat waves. Conclusion : Certain drugs should be administered with caution in the elderly during heat waves. Prevention by means of decreased drug dosages, changing or discontinuing certain drugs should be considered on a case by case basis. Humans are warm-blooded mammals and must maintain a constant internal temperature. Body temperature is controlled by the hypothalamus in the brain, with input from sensory receptors throughout the body. At any given time, body temperature is a function of heat production and heat loss. Temperature-regulation problems can result in several conditions. These include fever, hyperthermia and hypothermia. EMTs and paramedics must be familiar with the physiological and pathophysiological processes resulting from these disorders. By close observation of the patient, it should be fairly easy to determine which underlying processes are causing the observed signs and symptoms. With this knowledge, you can provide competent, compassionate prehospital care.
