This essay seeks to lay the foundation for an understanding of welfare state retrenchment. Previous discussions have generally relied, at least implicitly, on a reflexive application of theories designed to explain welfare state expansion. Such an approach is seriously flawed. Not only is the goal of retrenchment (avoiding blame for cutting existing programs) far different from the goal of expansion (claiming credit for new social benefits), but the welfare state itself vastly alters the terrain on which the politics of social policy is fought out. Only an appreciation of how mature social programs create a new politics can allow us to make sense of the welfare state's remarkable resilience over the past two decades of austerity. Theoretical argument is combined with quantitative and qualitative data from four cases (Britain, the United States, Germany, and Sweden) to demonstrate the shortcomings of conventional wisdom and to highlight the factors that limit or facilitate retrenchment success.

Policy analysis is an established discipline in the industrialized world, yet its application to developing countries has been limited. The health sector in particular appears to have been neglected. This is surprising because there is a well recognized crisis in health systems, and prescriptions abound of what health policy reforms countries should introduce. However, little attention has been paid to how countries should carry out reforms, much less who is likely to favour or resist such policies. This paper argues that much health policy wrongly focuses attention on the content of reform, and neglects the actors involved in policy reform (at the international, national sub-national levels), the processes contingent on developing and implementing change and the context within which policy is developed. Focus on policy content diverts attention from understanding the processes which explain why desired policy outcomes fail to emerge. The paper is organized in 4 sections. The first sets the scene, demonstrating how the shift from consensus to conflict in health policy established the need for a greater emphasis on policy analysis. The second section explores what is meant by policy analysis. The third investigates what other disciplines have written that help to develop a framework of analysis. And the final section suggests how policy analysis can be used not only to analyze the policy process, but also to plan.

The hospitals, 1800-1948 : a study in social administration in England and Wales

The Beveridge Committee had an inauspicious start. It was intended by the British government to be a minor tidying-up operation. When the report was completed, the government seriously considered not publishing it. The report advocated family allowances, a free health service and full employment, and that all social insurance benefits should be at flat rate and at subsistence level with the aim of abolishing poverty. But the actual recommendations would not have fully achieved Beveridge's aims. Although the report had a rapturous media reception, the government initially decided not to commit itself to action. But pressure from parliament forced Prime Minister Churchill to produce plans for legislation. When the report finally became law, the benefits were about a third less than Beveridge had recommended. As a result his social assistance safety net, intended to have a very small role, ended up giving ultimate protection to some 7 million persons. The report had a major influence in other countries by setting a much more ambitious agenda for social security than had generally been accepted before.
