• Background: Diagnosis of chlamydial conjunctivitis is difficult in chronic diseases because chlamydial elementary bodies are mostly undetectable in conjunctival scrapings by cell culture. We therefore compared two nonculture antigen tests and three different serotests for anti-chlamydial antibodies with McCoy cell culture, the “gold standard” of chlamydial diagnosis. Conjunctival scrapings and serum samples of 93 patients attending the outpatient eye clinic in Graz because of chronic follicular conjunctivitis were tested.

In der vorliegenden Ubersicht wird die aktuelle Therapie der bakteriellen Keratitis unter besonderer Berucksichtigung der Chinolone der 4. Generation behandelt. Weiterhin wird auf die Bedeutung additiver Steroide eingegangen. Letztere mussen niedrig dosiert titriert und ausgeschlichen werden, wahrend Antibiotika hoch dosiert appliziert und abrupt abgesetzt werden.

Therapie und Prognose der bakteriellen Keratitis

Diese Fallbeschreibung dokumentiert eine klinisch signifikante, mit Ulcera marginalia einhergehende Infektion der Cornea des linken Auges bei einem 27 Jahre alten Patienten. Das Krankheitsbild trat akut auf, begleitet von einer Beeintrachtigung des Allgemeinbefindens und Serokonversion bezuglich der Chlamydien-KBR. Das diagnostische und therapeutische Vorgehen und die Aetiologie der Infektion werden diskutiert.

Einseitige Keratitis verursacht durch einen Chlamydia Trachomatis-Subtyp

50 Patienten, die das typische klinische Bild einer Chlamydien-Keratokonjunktivitis zeigten, wurden in diese Studie einbezogen. Im Direktabstrich wurden bei allen mittels direkter Immunfluoreszenz zahlreiche Elementarkorperchen nachgewiesen, welche fur Chlamydia trachomatis spezifisch sind. Alle Proben wurden zusatzlich mittels der Standard-Giemsa-Farbung und einer Zuchtung auf der McCoy-Zellkultur untersucht. Weiters bestimmten wir bei allen Patienten die Antikorper-Titer im Serum mit Hilfe des Immuno-Peroxidase-Tests. Ziel unserer Untersuchung war es, die Ergebnisse dieser derzeit gangigen, aber methodisch so verschiedenen Tests zu vergleichen und damit eine — nicht nur fur unseren Routinebetrieb — optimale Diagnostik zu etablieren. Between January and December 1990 a total of fifty patients presented at the outpatient clinic for infectious eye diseases of the 2. Department of Ophthalmology, University of Vienna, with the signs of a typical keratoconjunctivitis caused by Chlamydia trachomatis. This report describes all different laboratory methods availiable to confirm the diagnosis. Separate samples harvested from all eyes included in the study were examined with Giemsa’s staining and cultivated in cycloheximide-treated McCoy cells. Furthermore we performed a culture-independent diagnosis of C. trachomatis using monoclonal antibodies. The results of serological examinations including IgA and IgG are presented and discussed as well.

Über die Wertigkeit verschiedener Labormethoden in der Diagnostik okulärer Chlamydien-Infektionen

An der 2. Univ.-Augenklinik wurden in den letzten 2 Jahren 34 Falle von Ophthalmia neonatorum diagnostiziert, die durch Chlamydia trachomatis verursacht wurden. Die betroffenen Sauglinge waren zwischen 7 und 27 Tagen alt und zeigten das klinische Bild einer beidseitigen, eitrigen papillaren Konjunktivitis. Zwei Kinder hatten zum Zeitpunkt unserer Diagnosestellung bereits eine Pneumonie entwickelt, 24 litten an einer ausgepragten Rhinitis und Pharyngitis. Die Ubertragung des Erregers auf das Neugeborene erfolgt wahrend des Geburtsaktes durch die primar infizierte Mutter, wobei die Credesche Prophylaxe gegen diese Erreger wirkungslos bleibt. Das infektiose Agens kann wahrend der Geburt oder wahrend der nachfolgenden manifesten Infektion geschluckt oder aspiriert werden und dann entsprechende Folgesymptome (Pneumonie oder Gastroenteritis) verursachen. Diese Tatsache unterstreicht die enorme Wichtigkeit der systemischen Therapie bei allen Neugeborenen mit dieser Diagnose, da die Entwicklung einer Sepsis beim Neugeborenen innerhalb von Stunden moglich ist, wie auch in der Literatur beschrieben wird. Die Diagnose beim Saugling wird klinisch aufgrund der Inkubationszeit und mit Hilfe des direkten Immunfluoreszenztestes gestellt. Die Therapie erfolgt lokal und immer auch systemisch. Es mussen beide Elternteile unbedingt in die Behandlung einbezogen werden. Das klinische Bild, die Diagnostik sowie die richtige Therapie werden vorgestellt und diskutiert. At the second department of Ophthalmology of the University of Vienna 34 cases of ophthalmia neonatorum caused by Chlamydia trachomatis were diagnosed during a 2-year period. The bacteria could be detected by the direct immunofluorescent test using monoclonal antibodies. The afflicted babies were between 7 and 27 days old and showed the typical clinical features of a binocular, suppurativ papillar conjunctivitis within the specific incubation period of 6–21 days. 2 infants had already developed a pneumonia, 24 were affected with a distinct rhinitis and pharyngitis. The infection has happened during the birthact caused by the primarily infected mother. Credes prophylaxis has no effect on this bacterium. The typical clinical findings, the adequate diagnosis, and the proper treatment of the infants as well as their parents are presented and discussed.

Über die Bedeutung der okulären Chlamydieninfektionen beim Neugeborenen

During the past five years a presumptive diagnosis of ocular chlamydial disease has been made in 90 patients consulting the outpatient department of Freiburg University Eye Clinic. The diagnosis was confirmed by different methods (giemsa stains, chlamydia cultures and serology, electron microscopy and histology) in 27 cases. The clinical spectrum included inclusion blennorrhea in newborns as well as inclusion conjunctivitis, TRIC keratoconjunctivitis and sporadic trachoma in adults. Presumably the incidence of the disease is much higher than indicated by our figures. We suspect a very high number of undiagnosed cases. These patients usually do not receive proper therapy (tetracyclin or erythromycin for at least three weeks) and their disease may run a protracted course with the risk of permanent eye damage.
