Ectopic prostatic tissue was found in the spleen in a 49-year-old white man who died of wide-spread malignant mesothelioma. The prostatic origin of the tissue was affirmed by positive immunohistochemical staining for prostatic specific antigen and prostatic acid phosphatase.

The case of a 69-year-old man with a multicystic splenic tumor was reported. The tumor occupied about half of the spleen. It was composed of multiple cysts and histologically confirmed as a cyst adenocarcinoma. Invasion was observed in the splenic flexure of the colon. A small portion of the pancreatic tail was also involved in the tumor. As to the histogenesis of this “splenic cystic tumor”, two possible viewpoints, that is, a growth as a secondary metastasis of a cystadenocarcinoma of the pancreatic tail, and a development on the basis of an aberrant pancreatic tissue in the spleen, were proposed. Regardless of its histogenetical aspects, such a splenic lesion as the present case seems to occur extremely rarely, and only one instance similar to our case could be found in the literature.

A mucinous cystadenocarcinoma along with remnants of normal pancreatic tissue was discovered in the spleen of a 21-year-old woman. Extensive clinical, radiological, ultrasound and computed tomography investigation revealed that the patient's pancreas and ovaries were unremarkable. We presume an origin from heterotopic pancreatic tissue for this neoplasm and present a review of the literature in this regard.

A cystadenoma of the pancreas within the spleen from pancreatic heterotopia is described. The tumor was located in the hilum of the spleen and completely separated from the normal pancreatic tail. This fact strongly supports the possibility that this tumor arose from pancreatic heterotopia.

A 30-year-old female was referred to our hospital for further examination of liver dysfunction. A huge, soft mass was noted in her left upper quadrant on physical examination. Abdominal ultrasonography and computed tomography revealed a huge cystic tumor of 20 cm in the hilus of the spleen. Serum CA19-9 was 491 U/ml, and splenectomy was performed under suspicion of a malignant cystic tumor. The inner surface of the cyst was lined by squamous epithelial cells that were immunohistochemically positive for CA19-9. Serum CA19-9 level was normalized after the surgery. Our case of a very rare, huge epidermoid cyst of the spleen suggests that measurement of the serum CA19-9 level is useful for evaluating therapeutic efficacy of a splenic epidermoid cyst.
