Parents’ perspectives of physical activity participation among Canadian adolescents with Autism Spectrum Disorder

PURPOSE
To review the feasibility, acceptability, and effects of physiotherapy when delivered remotely.


MATERIALS AND METHODS
CINAHL, MEDLINE, EBM Reviews, and Cochrane Library databases (January 2015-February 2022) were searched and screened for papers (of any design) investigating remote physiotherapy. Data were extracted by two independent raters. Methodological quality of the identified papers was not assessed. Thematic content analysis drew out the key issues.


RESULTS
Forty-one papers (including nine systemic reviews and six with meta-analyses) were selected involving musculoskeletal, stroke and neurological, pulmonary, and cardiac conditions. The most commonly delivered intervention was remote exercise provision, usually following assessment which was completed in-person. All studies, which assessed it, found that remote physiotherapy was comparably effective to in-person delivery at lower cost. Patient satisfaction was high, they found remote physiotherapy to be more accessible and convenient. It boosted confidence and motivation by reminding patients when and how to exercise but adherence was mixed. No adverse events were reported. Barriers related to access to the technology; technical problems and concerns about therapists' workload.


CONCLUSIONS
Remote physiotherapy is safe, feasible, and acceptable to patients. Its effects are comparable with traditional care at lower cost.IMPLICATIONS FOR REHABILITATIONRemote physiotherapy is safe, feasible, and acceptable to patients with comparable effects to in-person care.Remote delivery increases access to physiotherapy especially for those who cannot travel to a treatment facility whether due to distance or disability.Remote physiotherapy may increase adherence to exercise by reminding patients when and how to exercise.Remote physiotherapy does not suit everyone, thus a hybrid system with both in-person and remote delivery may be most effective.

A scoping review of the feasibility, acceptability, and effects of physiotherapy delivered remotely.

Digital exercise therapies (DET) have the potential to bridge existing care gaps for people living with chronic conditions. Acting as either a standalone, embedded within multi-modal lifestyle therapy, or adjunct to pharmacotherapy or surgery, evidence-based DETs can favorably impact the health of a rapidly growing population. Given the nascent nature of digital therapeutics, the regulatory landscape has yet to mature. As such, in the absence of clear guidelines clinical digital product developers are responsible for ensuring the DET adheres to fundamental principles such as patient risk management and clinical effectiveness. The purpose of this narrative review paper is to discuss key considerations for clinical digital product developers who are striving to build novel digital therapeutic (DTx) solutions and thus contribute towards standardization of product development. We herein draw upon DET as an example, highlighting the need for adherence to existing clinical guidelines, human-centered design and an intervention approach that leverages the Chronic Care Model. Specific topics and recommendations related to the development of innovative and scalable products are discussed which ultimately allow for differentiation from a basic wellness tool and integration to clinical workflows. By embodying a code of ethics, clinical digital product developers can adequately address patients' needs and optimize their own future digital health technology assessments including appropriate evidence of safety and efficacy.

Exercise therapy as a digital therapeutic for chronic disease management: consideration for clinical product development

The aim of this research is to examine the distance education experiences of physical education and sports teachers during the Covid-19 pandemic process. Eleven (7 Male, 4 Female) physical education and sports teachers who were determined by criterion sampling, one of the non-random sampling methods, participated in this study, which was designed in the descriptive qualitative methodology model. Semi-structured interview form was used in the research. Interview data were analyzed thematically. In this study, which examined the experiences of physical education and sports teachers towards the distance education model, 5 (five) themes were reached by analyzing the data collected from the participants: 1) General opinions, 2) Positive sides, 3) Negative sides, 4) Obstacles and 5) Suggestions.  In addition, fourteen (14) sub-themes were obtained within the scope of these themes. As a result, it was determined that physical education and sports teachers supported the decision to close the schools but did not rely on the distance education model. Although the participants acknowledged that there was a sudden transition to distance education due to the pandemic, it was observed that they generally focused on the negative sides. Participants stated that applied lessons such as physical education and sports were not very efficient with the distance education model. In the study, physical education and sports teachers stated that as the positive side of distance education, theoretical lessons have become important and the use of educational technology has increased. In addition, the participants made suggestions for the solution of some negative situations experienced in physical education and sports lessons conducted with the distance education model. It was observed that these suggestions focused on the technological development of teachers and increasing the support provided by the state.

Technology Update in Management of Multi-Ligament Knee Injuries

Purpose: We explored patients' perspectives on home exercise programmes (HEPs) and their experiences using a mobile application designed to facilitate home exercise. Method: Data were generated using qualitative, semi-structured, face-to-face interviews with 10 participants who were receiving outpatient physiotherapy. Results: Establishing a therapeutic partnership between physiotherapists and patients enabled therapists to customize the HEPs to the patients' lifestyles and preferences. Analysis suggests that using the mobile application improved participants' ability to integrate the HEP into their daily life and was overwhelmingly preferred to traditional paper handouts. Conclusions: The results suggest that efforts to engage patients in HEPs need to take their daily lives into account. To move in this direction, sample exercise prescription questions are offered. Mobile applications do not replace the clinical encounter, but they can be an effective tool and an extension of delivering personalized HEPs in an existing therapeutic partnership.
