Background Globalisation is the process by which traditional boundaries of cultures are changing. Industrialisation, urbanisation and influence of the media are influencing idioms of distress across cultures.

Aims To discuss the role of globalisation, using the epidemiology, diagnosis, clinical presentation and treatment of depression across various cultures as an example.

Method Recent studies focusing on transcultural aspects of depression were reviewed and summarised.

Results Cultural, social and religious mores account for variations in the presentation of depression across cultures. Somatic symptoms are common presenting features throughout the world and may serve as cultural idioms of distress, but psychological symptoms can usually be found when probed. Feelings of guilt and suicide rates vary across cultures and depression may be underdiagnosed.

Conclusions Training packages could enhance clinicians’cultural competency in multicultural settings. However, globalisation is likely to influence idioms of distress and pathways to care in ways that are difficult to predict.

Depression as a disorder has always been a focus of attention of researchers in India. Over the last 50-60 years, large number of studies has been published from India addressing various aspects of this commonly prevalent disorder. The various aspects studied included epidemiology, demographic and psychosocial risk factor, neurobiology, symptomatology, comorbidity, assessment and diagnosis, impact of depression, treatment related issues and prevention of depression in addition to the efficacy and tolerability of various antidepressants. Here, we review data on various aspects of depression, originating from India.

An overview of Indian research in depression

This article summarizes the findings and theoretical perspectives of 19 empirical studies of sexual problems in gay men. In order to understand these problems better, various differences between male homosexual and heterosexual functioning are discussed first. The studies included differ widely in terms of the issues explored, the populations studied, and the way data have been collected. In a few studies, researchers generally have looked at the prevalence and experience of sexual problems. In other studies, researchers have focused on the etiology and treatment of specific problems, such as sexual desire disorders, sexual aversion, excitement and arousal problems, orgasm disorders, sexual pain disorders and sexual compulsivity. Overall it is surprising how little is known about these problems in gay men. This is also true, however, for same-sex sexuality in general. Suggestions are made for studies that will enable us to obtain a better understanding of sexual problems in gay men.

Sexual problems in gay men: an overview of empirical research.

This article reviews and critiques couples therapy research that has focused on gay and lesbian relationships. In conducting our examination, we felt that it was imperative to address the sociopolitical climate that affects same-sex couples and that historically impeded this area of research. Upon addressing the therapist's role and comparing heterosexual relationships to same-sex relationships, this article presents the theoretical formulations and empirical findings that have informed gay and lesbian couples therapy research. We also present limitations of the current literature and suggestions for future research.

Culture and Mental Illness: Social Context and Explanatory Models

Cross-cultural studies of depression have raised interesting issues in terms of symptoms and diagnosis in patients seeking treatment. To understand the symptom patterns of depression in a clinic population in north India, we studied 75 consecutive patients diagnosed as suffering from depression. They were asked to describe their symptoms, life events in the year before the onset of depression, and family history. Sleep disturbances, low mood and irritability were common symptoms. Suicidal intent was reported by 54% of the sample, and guilt by 56%. Study of family history and life events suggests that in those with a negative family history, the onset of depression was more likely to be related to a significant life event the commonest being family conjunct. The symptoms of this patient group are compared with the symptoms of depression reported from clinical samples in south India, Bombay and the West.
