Measures of Need and Outcome for Primary Health Care

PurposeThe purpose of this study is to identify medical service quality factors that patients care about and establish a medical service quality evaluation index system by analyzing online reviews of medical and healthcare service platforms in combination with a questionnaire survey.Design/methodology/approachThis study adopts a combination of review mining and questionnaire surveys. The latent Dirichlet allocation (LDA) model was used to mine hospital reviews on the medical and healthcare service platform to obtain the medical service quality factors that patients pay attention to, and then the questionnaire was administered to obtain the relative importance of these factors to patients' perception of service quality. Finally, the index system was established.FindingsThe medical service quality factors patients care about include medical skills and ethics, registration service, operation effect, consulting communication, drug therapy, diagnosis process and medical equipment.Research limitations/implicationsThe identification of medical service quality factors provides a reference for medical institutions to improve their medical service quality.Originality/valueThis study uses online review mining to obtain medical service quality factors from the perspective of patients, which is different from previous methods of obtaining factors from relevant literature or expert judgments; then, based on the mining results, a medical service quality evaluation index system is established by using questionnaire data.

Research on the identification of medical service quality factors: based on a data-driven method

The purpose of the present study is to provide empirical evidence on the adaptation of the mathematics-related beliefs questionnaire (MRBQ) supplemented by info-communication technology-related items. Besides, we also investigate the relationship between their beliefs about mathematics and their ability in problem-solving mathematics. 234 grade eight students from five schools in Java, Indonesia, participated in this study. The questionnaire has appropriate reliability. To examine the validity of the questionnaire, factor analysis was applied, and regression analysis was conducted to explore students’ beliefs and their relation to their performance. Factor analysis revealed a good fit of the model; therefore, confirming the validity of MRBQ in the Indonesian context. Descriptive statistics showed students’ tendency to follow the nonrealistic approach when doing word problems. Regression analysis indicated the significant role of beliefs in mathematics predicting students’ performance on mathematical word problems.

Development and application of an in-house health care program to improve the physical and mental health of working mothers: A pilot study

Background There is a shortage of rural general practitioners (GPs) in China. Training programs have been initiated to meet the needs of GP workforce in the rural areas; however, there is an absence of validated tool to assess their competencies. Objective This study aimed to develop a competency model for rural general practitioner (CMRGP) after training in China and to examine its validity and reliability. Methods A multistage process was adopted to develop the CMRGP comprised literature review, panel discussion and expert consultation, and the initial version of CMRGP was reduced from 10 domains and 77 items to 7 domains and 54 items. A pilot study was conducted among 202 rural GPs for the psychometric evaluation and application of the initial version of CMRGP, in which a questionnaire on the importance of items and self-evaluation was completed by the GPs. Results In the pilot study, 132 completed questionnaires (65.3%) were returned. Acceptability and face validity of the CMRGP were supported by high importance scores of the items, in which 52 out of 54 items achieved score higher than 4.00 (possible score from 0 to 5). Factor analysis supported the construct validity. After the modification, the final version of CMRGP contained 6 domains and 47 items. Good reliability was supported by internal consistency reliability (Cronbach's α was 0.98) and split-half reliability (Spearman-Brown coefficient was 0.99). Conclusions The CMRGP demonstrated good reliability and validity. Pilot study showed its potential for application in the rural general practice and training program.

Development and preliminary validation of the competency model for rural general practitioner in China.

Background and aims Patient satisfaction is one of the important ways to measure the results of treatment and the quality of medical services. The purpose of the present study was to verify the reliability and validity of the indicator system for evaluating satisfaction of outpatients and inpatients in Chinese hospitals. Methods The study was based on the satisfaction questionnaire program of the national doctor- patient experience research center with data from 99,802 outpatients and 229,215 inpatients, collected in China between 2016 and 2017. We adopted exploratory factor analysis for validity analysis and the method of split-half reliability and Cronbach's α coefficient for reliability analysis. Results In the validity analysis of the indicator system for outpatients, the factor loading was between 0.438 and 0.919, and the reliability was excellent. In the validity analysis of the indicator system for nonsurgical inpatients, the factor loading of the secondary indicators was between 0.417 and 0.75, and the reliability was excellent. In the validity analysis of the indicator system for surgical inpatients satisfaction, the factor loading of the secondary indicators range was 0.391-0.751, and the reliability was excellent. Conclusion The indicator systems for evaluating satisfaction of outpatients, surgical inpatients and nonsurgical inpatients all have excellent reliability and good validity. They can be widely used for an outpatient and inpatient satisfaction questionnaire in Chinese hospitals.
