Cystic lymphangioma is a rare, benign malformation of the lymphatic vessels which may be observed on various locations. Retroperitoneal location is less common than mesenteric location. Cystic lymphangioma has a polymorphic clinical presentation. Diagnosis is based on imaging but requires histological confirmation. Surgery is the treatment of choice. The aim of our study is to analyze the clinical manifestations, complications, diagnostic and therapeutic aspects of this tumor. We report a case series of 5 patients with retroperitoneal cystic lymphangioma (4 women and 1 man) operated in our department between the years 2004 and 2014. Their medical records were reviewed retrospectively. Follow-up was based on clinical examination and abdominal CT scan. The average age was 45 years. The mean follow-up was 32.6 months. The most common symptoms indicative of retroperitoneal cystic lymphangioma were pains and/or an abdominal mass. Abdominal CT scan was the most useful diagnostic test. Total resection was immediately achieved in 4 patients and it was deferred for up to 5 years in one patient. He underwent annual ultrasound monitoring. One patient underwent nephrectomy. No recurrence or complications were noted in 5 patients. Retroperitoneal cystic lymphangioma is a rare condition. Its therapeutic management is based on complete resection in patients with symptomatic lesions or complications, in order to limit the risk of recurrence. Complete resection may be deferred in asymptomatic patients.

Cystic lymphangioma is a rare benign malformative tumor of lymphatic vessels in various locations.
We report the observation of a 62-year-old man who had been consulting for major abdominal distention for 5 years. The clinical examination found a very distended abdomen with collateral venous circulation.
The imaging was in favor of a cystic retro-peritoneal image. Surgical exploration showed a retroperitoneal cystic mass. Anatomo-pathological examination concluded that retroperitoneal cystic lymphangioma.

Le lymphangiome kystique est une tumeur bénigne malformative rare des vaisseaux lymphatiques à localisations diverses. La localisation rétropéritonéale est moins fréquente comparée à celle mésentérique. Sa présentation clinique est polymorphe. Le diagnostic est évoqué par l'imagerie mais il nécessite une confirmation histologique. Le traitement de choix est chirurgical. Notre objectif est d'étudier les manifestations cliniques, les complications, les aspects diagnostiques et thérapeutiques de cette tumeur. Nous rapportons une série de 5 cas de lymphangiomes kystiques rétropéritonéaux (4 femmes et un homme) opérés dans notre service entre les années 2004 et 2014. Leurs dossiers ont été examinés rétrospectivement. Le suivi était basé sur l'examen clinique et l'échographie abdominale. L´âge moyen était de 45 ans. Le suivi moyen était de 32,6 mois. La symptomatologie révélatrice la plus fréquente était les douleurs et/ou une masse abdominale. Le scanner abdominal était l'examen le plus utile au diagnostic. Une exérèse complète était réalisée d'emblée chez quatre patients et elle était différée après cinq ans de surveillance par une échographie annuelle chez un. Dans un cas, on a eu recours à une néphrectomie. Aucune récidive ni complication n´ont été notées chez les 5 patients. le lymphangiome kystique à localisation rétropéritonéale est une affection rare. Sa prise en charge thérapeutique repose sur une exérèse complète, de cas de lésions symptomatiques ou de complications, pour limiter le risque de récidive. Cette dernière peut être différée chez les patients asymptomatiques.

Gastrointestinal Stromal Tumors Treated by Mesylate Imatinib to the Teaching Hospital of Brazzaville

Les tumeurs stromales gastro-intestinales (GIST, gastro-intestinal stromal tumor) sont des tumeurs rares dont l’individualisation a ete possible grâce aux techniques d’immunomarquage, et dont le pronostic a ete considerablement ameliore grâce aux antityrosines-kinases. Leurs caracteristiques sont peu etudiees en Afrique subsaharienne. Notre etude decrit les caracteristiques cliniques et pronostiques de 15 cas de GIST dans un service d’oncologie medicale subsaharien a l’ere des antityrosines-kinases. Ces cas representaient 4,3 % des tumeurs digestives et affectaient 11 hommes et quatre femmes, dont l’âge moyen etait de 62 ± 9,9 ans. Leurs circonstances de diagnostic etaient dominees par la douleur epigastrique (67 %) et le syndrome occlusif (13 %), refletant leurs localisations surtout gastriques (53 %) et greliques (20 %). Les types histologiques frequents etaient la forme fusiforme (80 %) et la forme epithelioide (13 %). Toutes les tumeurs exprimaient le CD117 et avaient un index mitotique superieur a 5/50 CFG (Champs a Fort Grossissement), trois exprimaient en plus le DOG1. La tumeur etait metastatique et non operable chez sept patients, operee chez les huit autres. Un traitement par imatinib mesylate a permis d’obtenir une remission complete chez les huit patients operes, une stabilisation des lesions chez deux patients non operes avec un recul de suivi de 2 a 96 mois. Cinq cas metastatiques avaient progresse avec une issue fatale pour quatre patients. En conclusion, les GIST ont, dans notre contexte, des localisations le plus souvent gastriques ou greliques, sans specificite clinique. Leur sensibilite a l’imatinib mesylate incite a les evoquer devant toute symptomatologie digestive chez le sujet âge de plus de 40 ans.

Resume Bien qu’elles soient rares, les tumeurs stromales gastro-intestinales (GIST), sont les tumeurs mesenchymateuses les plus frequentes du tube digestif (Mignon et al. [1] ), souvent de localisation gastrique, antrale et fundique, rarement cardiale. L’objectif de notre travail est d’etudier la localisation cardiale exceptionnelle de ces tumeurs, qui pose a la fois un probleme diagnostique et therapeutique. Nous rapportons le cas d’une patiente âgee de 54 ans, sans antecedents particuliers qui a presente quatre mois avant son admission une dysphagie aux solides d’installation progressive associee a un amaigrissement non chiffre. Le bilan paraclinique a montre l’existence d’une lesion du mediastin inferieur tres probablement a point de depart digestif. En peroperatoire ; il s’agit d’une tumeur de la jonction œso-gastrique. Une œso-gastrectomie polaire superieure sans curage ganglionnaire est realisee. Le complement immuno-histochimique etait en faveur d’une tumeur stromale de la jonction œso-gastrique.
