Each year there has been a progressive decrease in the number of procedures performed; the incidence of adenoidectomy has declined particularly. Although infection remains the predominant indication for surgery, there has been a dramatic rise in obstructive sleep apnea as a significant indication, from 0% in 1978 to 19% in 1986

Tumours of the salivary glands are unusual in the dog, with an overall incidence reported as 0.2%. This chapter looks at clinical approach; management; prognosis.

Tumours of the salivary glands

Between 1991 and 2006, 684 cases of salivary gland tumours were analysed retrospectively, of which 422 (62%) were benign and 262 (38%) malignant. Sixty-one percent of tumours were in the parotid gland, 22% in the minor salivary glands, and 17% in the submandibular glands. The most common benign tumour was pleomorphic adenoma (86%), and the most common malignant tumours were adenoid cystic carcinoma (25%) and mucoepidermoid carcinoma (18%). Among the minor salivary gland tumours, most were seen in the palate (68%). We analyse the incidence and distribution of all types of salivary gland tumours in an Indian series, and provide data for comparison with other epidemiological studies from different geographical sites and races. Demographic data from these studies should help us to a better understanding of the biological and clinical characteristics of the disease.

Salivary gland tumors: a single institution experience in India

Background: The incidence of salivary gland tumors is claimed to be influenced by geographical and racial factors. The pathological classification and nomenclature of salivary gland tumors as defined by WHO classification (1991), is accepted world-wide but little is available in the literature regarding the spectrum of salivary gland tumors in Africa in the basis of this classification. Such efforts would allow comparison and justify any differences between the black African population and the rest of the world. Objective: To outline the clinicopathological features of salivary gland tumors in Uganda. Setting: Makerere University, Faculty of Medicine, Department of Pathology. 
Methods: All epithelial tumors from major and minor salivary glands accessioned from 1979 to 1988 were analyzed in respect to sex and age of patients, anatomical location of the tumor and histological type. The histological diagnosis of each individual tumor was based on the 1991 WHO classification of salivary gland tumors. Results: During the span of 10 years, 268 cases of salivary gland tumors were diagnosed. Of these, 113 (42.2%) were males, 148 (55.2%) females and in the remaining seven (2.6%) cases, the sex was not specified. The age range of the 247 patients with recorded ages was from 0.5 to 80 years. The mean age at diagnosis was 38.1 (SD =17.03) with the median of 38.0 years. Thirty four percent of tumors originated from the parotid, 33.2% from the submandibular and 32.8% from minor salivary glands. No tumor was implicated from the sublingual gland. There were a total of 125 (46.6%) malignant tumors and 143 (53.4%) benign tumors. The mean age of patients with malignant lesions (43.1 years; SD=16.75; median=44.00 years) was 9.6 years older than those with benign tumors (mean=33.5 years; SD=16.0; median=30.00 years). Pleomorphic adenoma was the most common benign tumor (74.8%), followed by myoepithelioma (9.8%). No Whartin's tumor was encountered. The malignant tumors were dominated by adenoid cystic carcinoma (28.8%) followed by mucoepidermoid carcinoma (21.6%). 
Conclusion: The pattern of distribution of salivary gland tumors in black African population seems to differ from that of Western series in that; i) females are more affected than males, ii) there is a low proportion of tumors from the parotid gland and high proportion of tumors from the submandibular and minor salivary glands, iii) the parotid and minor salivary gland tumors have more probability of being malignant than those tumors from the submandibular gland iv) the newly categorized pathological entities are common and v) Whartin's tumor is extremely rare in black African population. Salivary gland tumors in Uganda: clinical pathological study

Background: Although adenoidectomy is generally applied in the treatment of otitis media with effusion (OME), there is still much debate about the role of adenoid in the pathogenesis of OME. The purpose of this study is to determine the incidence of OME in children with obstructive adenoid disease in comparison with normal control, and the degree of nasopharyngeal obstruction by adenoid as it relates to the development ofOMEin Nigerian children. Method: Controlled, prospective clinical study was carried out. Diagnosis of OME was made with finding of type B tympanogram on tympanometry evaluation. The incidence of OME among adenoidal patients was compared with its incidence in normal control. The degree of nasopharyngeal obstruction among the adenoidal subjects was evaluated with an adenoidal-nasopharyngeal ratio parameter obtained from soft tissue radiograph of nasopharynx, and was related to the results of tympanometric evaluation of the adenoidal subjects. Results: The incidence of OME was significantly higher in the adenoidal children than the normal control (p < 0.001). The risk ofOMEwas more than 7 times as more among adenoidal group than among the non-adenoidal control. Gross nasopharyngeal obstruction was significantly associated with type B tympanogram (p= 0.002). The diagnosis of OME correlated significantly with the degree of nasopharyngeal obstruction (r = 0.32; p = 0.002).Our study found adenoid obstruction as a significant risk factor for OME in children. The role of adenoidal obstruction in the pathogenesis of otitis media with effusion in Nigerian children.

Background:Obstructive adenoid enlargement is commonly implicated as the major cause of chronic nasal obstruction in children. Although clinical assessment is considered essential, there is little consensus over its reliability. This study was conducted to determine the correlation between graded symptomatology assessment and roentgenographic assessment of adenoidal obstruction.Method:Symptoms assessed included snoring, mouth-breathing and obstructive breathing during sleep. Each symptom was rated on a four-point scale (absent = zero, mild = one, moderate = two and severe = three). We summed the ratings for each child to obtain the symptomatology score. We used an adenoidal–nasopharyngeal ratio parameter to classify roentgenographic assessment into minimal, moderate or marked obstruction.Results:Sixty-four children, 42 boys and 22 girls, aged one to 12 years were enrolled. The clinical symptomatology scores correlated significantly with the roentgenographic ratings of nasopharyngeal airway obstruction (r = 0.419; p = 0.001). The correlation was significant at roentgenographic ratings of minimal obstruction (p < 0.05) and gross obstruction (p < 0.001). Both the symptomatology score and the roentgenographic rating showed significant correlations with patient age (r = −0.657, p < 0.01 and r = −0.340, p < 0.01, respectively).Conclusion:Clinical rating of adenoidal symptoms in children provides a reasonably reliable assessment of the presence and severity of nasopharyngeal airway obstruction. This technique of assessment is easy to use and is particularly valid when obstruction is either minimal or gross.

Evaluation of adenoidal obstruction in children: clinical symptoms compared with roentgenographic assessment

Background: Impacted wax has been classified as an ear disease. It can cause pain, itching, tinnitus hearing loss or otitis externa. The prevalence of cerumen impaction varies. The aim of this study was to determine the prevalence of impacted ear wax in primary school children and to determine, if there is any association between socioeconomic status and the occurrence of wax impaction among these school children. Methods: A multi-staged stratified sampling technique was used to select 630 pupils from 15 primary schools in Ile-Ife using the Local Education Authority (LEA) list for common entrance code as the sampling frame. Pre-tested structured questionnaire was administered on each selected pupil with clarification from parent/ guardian where necessary and were examined. Each pupil was placed in the upper, middle and lower socioeconomic class based on Oyedeji’s classification. Results: Three hundred and one (47.8%) were females and three hundred and twenty-nine (52.2%) were males. Forty-two point two percent (42.2%) of the pupils were from the lower socioeconomic class, 31.6% (199) were from the middle class and 26.6% (164) were from the upper class. Wax impaction had a prevalence of 46.7% and significantly higher in the lower socioeconomic status (p=0.036). Conclusion: Wax impaction is a problem amongst Nigerian primary school children. There is a significant relationship between wax impaction and low socioeconomic status. Regular otoscopy of school children is being advocated for early detection and subsequent treatment to prevent the attendant problem of hearing impairment which leads to poor school performance.

Wax Impaction in Nigerian School Children.

Four patients treated for cut-throat injuries are the subjects of this paper. Two were self-inflicted suicide attempts while one each was for ritual killing and over disputed farmland. Tracheostomy was deplored at the early period in all cases to ensure safe and patent airway. Significant laryngo-tracheal stenosis was a long-term morbidity suffered by two of the patients. Management of the airway in the early and late periods following the cut-throat injuries is discussed.

Management of the upper airway in severe cut-throat injuries.

The objective of this study is to describe the pattern of salivary gland neoplasms as experienced in Enugu-Nigeria. Patients are selected from those attending the Otorhinolaryngology units of the author at the University of Nigeria Teaching Hospital and Balsam Clinics both in Enugu from January 1992 to December 1997, with tumours of salivary gland origin. Non-neoplastic lesions were excluded. There were forty-one (41) patients with salivary gland neoplastic growths in the following sites: parotids (25) submandibular gland (SMG) (10) and minor (6). There were 15 males, 26 females; age range 10-74 year, with mean of 41 years. Seven of the parotid tumours were recurrent/residual. Salivary gland neoplasms are important surgical disease in this region with clinical manifestations similar to what obtains in other parts of the world. Surgical excision of salivary neoplasms is beneficial, but misadventure is still rampant.
