Wearable devices provide an alternative pathway to clinical diagnostics by exploiting various physical, chemical and biological sensors to mine physiological (biophysical and/or biochemical) information in real time (preferably, continuously) and in a non-invasive or minimally invasive manner. These sensors can be worn in the form of glasses, jewellery, face masks, wristwatches, fitness bands, tattoo-like devices, bandages or other patches, and textiles. Wearables such as smartwatches have already proved their capability for the early detection and monitoring of the progression and treatment of various diseases, such as COVID-19 and Parkinson disease, through biophysical signals. Next-generation wearable sensors that enable the multimodal and/or multiplexed measurement of physical parameters and biochemical markers in real time and continuously could be a transformative technology for diagnostics, allowing for high-resolution and time-resolved historical recording of the health status of an individual. In this Review, we examine the building blocks of such wearable sensors, including the substrate materials, sensing mechanisms, power modules and decision-making units, by reflecting on the recent developments in the materials, engineering and data science of these components. Finally, we synthesize current trends in the field to provide predictions for the future trajectory of wearable sensors.

End-to-end design of wearable sensors

Humans rely increasingly on sensors to address grand challenges and to improve quality of life in the era of digitalization and big data. For ubiquitous sensing, flexible sensors are developed to overcome the limitations of conventional rigid counterparts. Despite rapid advancement in bench-side research over the last decade, the market adoption of flexible sensors remains limited. To ease and to expedite their deployment, here, we identify bottlenecks hindering the maturation of flexible sensors and propose promising solutions. We first analyze challenges in achieving satisfactory sensing performance for real-world applications and then summarize issues in compatible sensor-biology interfaces, followed by brief discussions on powering and connecting sensor networks. Issues en route to commercialization and for sustainable growth of the sector are also analyzed, highlighting environmental concerns and emphasizing nontechnical issues such as business, regulatory, and ethical considerations. Additionally, we look at future intelligent flexible sensors. In proposing a comprehensive roadmap, we hope to steer research efforts towards common goals and to guide coordinated development strategies from disparate communities. Through such collaborative efforts, scientific breakthroughs can be made sooner and capitalized for the betterment of humanity.

Wearable Sensors-Enabled Human–Machine Interaction Systems: From Design to Application

Smart textiles for personalized healthcare

Highly stretchable strain sensors based on conducting polymer hydrogel are rapidly emerging as a promising candidate toward diverse wearable skins and sensing devices for soft machines. However, due to the intrinsic limitations of low stretchability and large hysteresis, existing strain sensors cannot fully exploit their potential when used in wearable or robotic systems. Here, a conducting polymer hydrogel strain sensor exhibiting both ultimate strain (300%) and negligible hysteresis (<1.5%) is presented. This is achieved through a unique microphase semiseparated network design by compositing poly(3,4‐ethylenedioxythiophene):polystyrene sulfonate (PEDOT:PSS) nanofibers with poly(vinyl alcohol) (PVA) and facile fabrication by combining 3D printing and successive freeze‐thawing. The overall superior performances of the strain sensor including stretchability, linearity, cyclic stability, and robustness against mechanical twisting and pressing are systematically characterized. The integration and application of such strain sensor with electronic skins are further demonstrated to measure various physiological signals, identify hand gestures, enable a soft gripper for objection recognition, and remote control of an industrial robot. This work may offer both promising conducting polymer hydrogels with enhanced sensing functionalities and technical platforms toward stretchable electronic skins and intelligent robotic systems.

High‐Stretchability, Ultralow‐Hysteresis ConductingPolymer Hydrogel Strain Sensors for Soft Machines

Soft machines are a promising design paradigm for human-centric devices1,2 and systems required to interact gently with their environment3,4. To enable soft machines to respond intelligently to their surroundings, compliant sensory feedback mechanisms are needed. Specifically, soft alternatives to strain gauges—with high resolution at low strain (less than 5 per cent)—could unlock promising new capabilities in soft systems. However, currently available sensing mechanisms typically possess either high strain sensitivity or high mechanical resilience, but not both. The scarcity of resilient and compliant ultra-sensitive sensing mechanisms has confined their operation to laboratory settings, inhibiting their widespread deployment. Here we present a versatile and compliant transduction mechanism for high-sensitivity strain detection with high mechanical resilience, based on strain-mediated contact in anisotropically resistive structures (SCARS). The mechanism relies upon changes in Ohmic contact between stiff, micro-structured, anisotropically conductive meanders encapsulated by stretchable films. The mechanism achieves high sensitivity, with gauge factors greater than 85,000, while being adaptable for use with high-strength conductors, thus producing sensors resilient to adverse loading conditions. The sensing mechanism also exhibits high linearity, as well as insensitivity to bending and twisting deformations—features that are important for soft device applications. To demonstrate the potential impact of our technology, we construct a sensor-integrated, lightweight, textile-based arm sleeve that can recognize gestures without encumbering the hand. We demonstrate predictive tracking and classification of discrete gestures and continuous hand motions via detection of small muscle movements in the arm. The sleeve demonstration shows the potential of the SCARS technology for the development of unobtrusive, wearable biomechanical feedback systems and human–computer interfaces. Strain gauges with both high sensitivity and high mechanical resilience, based on strain-mediated contact in anisotropically resistive structures, are demonstrated within a sensor-integrated, textile-based sleeve that can recognize human hand motions via muscle deformations.
