Background There is increasing pressure to tackle the wider social determinants of health through the implementation of appropriate interventions. However, turning these demands for better evidence about interventions around the social determinants of health into action requires identifying what we already know and highlighting areas for further development. Methods Systematic review methodology was used to identify systematic reviews (from 2000 to 2007, developed countries only) that described the health effects of any intervention based on the wider social determinants of health: water and sanitation, agriculture and food, access to health and social care services, unemployment and welfare, working conditions, housing and living environment, education, and transport. Results Thirty systematic reviews were identified. Generally, the effects of interventions on health inequalities were unclear. However, there is suggestive systematic review evidence that certain categories of intervention may impact positively on inequalities or on the health of specific disadvantaged groups, particularly interventions in the fields of housing and the work environment. Conclusion Intervention studies that address inequalities in health are a priority area for future public health research.

Tackling the wider social determinants of health and health inequalities: evidence from systematic reviews

In recent years, there has been an increasing interest in how organizational-level occupational health interventions aimed at improving psychosocial working conditions and employee health and well-being may be planned, implemented and evaluated. It has been claimed that such interventions have the best chance of achieving a significant impact if they follow an intervention process that is structured and also includes the participation of employees. This paper provides an overview of prominent European methods that describe systematic approaches to improving employee health and well-being through the alteration of the way in which work is designed, organized and managed. The methods identified are the Risk Management approach and the Management Standards from Great Britain, the German Health Circles approach, Work Positive from Ireland and Prevenlab from Spain. Conducting organizational-level occupational health interventions: What works?

Interventions that aim at improving health by changing the organization of work-in terms of task characteristics, work conditions, and social aspects-have shown their potential, but results are mixed, and many studies do not use their methodological potential. It is proposed that interventions at the organizational level are likely to have a more diverse effect than at the individual level, as the number of subsystems, with potentially diverging interests, is larger. Even well-implemented interventions are not likely to lead to improvements in all parameters for all participants, and trade-offs have to be considered. Methodological improvement is necessary but should not only focus on design issues, but also on careful documentation and subgroup analyses. A combination of person-focused and organization-focused approaches is the most promising. Finally, evidence points to the limited utility of economic arguments for the acceptance of health promotion projects; the necessity of professional trust is therefore emphasized.

Occupational musculoskeletal and mental health: Significance of rationalization and opportunities to create sustainable production systems ― A systematic review

Health circles, the central element of a comprehensive health promotion approach that has been developed in Germany in recent years, emphasize organizational and psychosocial factors while actively involving employees in the process. Through an extensive review the authors identified 11 studies, presenting the results of 81 health circles. The scientific quality of the data is limited: only 3 studies used (nonrandomized) control groups, whereas the remaining studies are based on retrospective before-and-after comparison. Nonetheless, the available data suggest that health circles are an effective tool for the improvement of physical and psychosocial working conditions and have a favorable effect on workers' health, well-being, and sickness absence. More rigorous studies are needed to confirm these results.
