Quantifying atherogenic lipoproteins for lipid-lowering strategies: consensus-based recommendations from EAS and EFLM

The familial social background of a child can significantly impact their behavior and health. We investigated associations between socio-economic status (SES) and health parameters and behaviors in German children and adolescents. Data were collected between 2011 and 2018 in the framework of the LIFE Child study. Participants included 2998 children aged 3⁻18 years. SES was represented by an index combining information on parental education, occupation, and income. Associations between SES and health outcomes were estimated using linear and logistic regression analyses. In a moderator analysis, all associations were checked for interactions between SES and age or sex. A higher SES composite score was associated with better health (lower body mass index (β = -0.26), fewer behavioral difficulties (β = -0.18), higher quality of life (β = 0.21), fewer critical life events (odds ratio (OR) = 0.93); all p < 0.05) and a healthier lifestyle (healthier nutrition (β = 0.16), less excessive television use (OR = 0.87), less nicotine consumption (OR = 0.93), and more physical activity (OR = 1.18); all p < 0.05). However, SES was not associated with alcohol consumption (OR = 1.02) or sleep problems (β = -0.04). The strengths of the associations between SES and child health did not differ depending on SES indicator (education, occupation, income). The associations between SES and parent-reported behavioral difficulties and physical activity were stronger in older vs. younger children. In contrast, none of the observed associations were moderated by sex. This study highlights the strong association between socio-economic status and child health, even in modern Western societies.

Background In terms of maternal morbidity and mortality, the puerperium is just as significant as pregnancy and childbirth. Nearly half of all maternal deaths occur in the time after delivery. Methods This review is based on pertinent articles in English and German from the years 2000- 2020 that were retrieved by a selective search in MEDLINE and EMBASE, as well as on the available guidelines in English and German and on German-language textbooks of obstetrics. Results The most common and severe complications are, in the post-placental phase, bleeding and disturbances of uterine involution; in the first seven days after delivery, infection (e.g., endomyometritis, which occurs after 1.6% [0.9; 2.5] of all births) and hypertension-related conditions. Thromboembolism, incontinence and disorders of the pelvic floor, mental disease, and endocrine disturbances can arise at any time during the puerperium. In an Australian study, the incidence of embolism was 0.45 per 1000 births, with 61.3% arising exclusively after delivery. Conclusion Basic familiarity with the most common and severe diseases in the puerperium is important for non-gynecologists as well, among other things because highly acute, lifethreatening complications can arise that demand urgent intervention.

Ziel Ziel der Studie war die Untersuchung von Einflussgrosen auf die
 sonographische Schatzgenauigkeit makrosomer Neugeborener. Studiendesign und Methoden In die Untersuchung wurden 224 schwangere
 Frauen im Alter von 16–43 Jahren eingeschlossen und deren fetale
 Gewichtsschatzungen (EFW) mit den Geburtsgewichten (BW) der Neugeborenen
 verglichen. Die Daten wurden monozentrisch und retrospektiv am
 Universitatsklinikum Leipzig fur 2014 erhoben. Eingeschlossen wurden
 Einlingsschwangerschaften mit einem tatsachlichen BW ≥ 4000 g. Ergebnisse Die Pravalenz der Makrosomie betrug 10 % (mittleres
 Geburtsgewicht 4270g). Mit 80,6 % wurden die makrosomen Feten sonographisch
 primar unterschatzt. Fur den mittleren Schatzfehler ∆ nach Merz und Hadlock
 sowie die Schatzgenauigkeit nach Entbindungsmodus und klinische Qualifikation
 des Untersuchers konnten keine Signifikanzen gezeigt werden. Fur das maternale
 Alter, die Gewichtszunahme in der Schwangerschaft, den maternalen BMI, das
 Vorliegen einer diabetischen Stoffwechsellage sowie die Fruchtwassermenge vor
 Entbindung konnten ebenfalls keine signifikanten Korrelationen zum Schatzfehler
 ∆ gezeigt werden. Schlussfolgerung Makrosome Neugeborene werden antenatal eher unterschatzt,
 wobei der mutterliche BMI den Schatzfehler nicht beeinflusst.
