Congenital heart disease is the most common congenital disorder in newborns. Advances in cardiovascular medicine and surgery have enabled most patients to reach adulthood. Unfortunately, prolonged survival has been achieved at a cost, as many patients suffer late complications, of which heart failure and arrhythmias are the most prominent. Accordingly, these patients need frequent follow-up by physicians with specific knowledge in the field of congenital heart disease. However, planning of care for this population is difficult, because the number of patients currently living with congenital heart disease is difficult to measure. Birth prevalence estimates vary widely according to different studies, and survival rates have not been well recorded. Consequently, the prevalence of congenital heart disease is unclear, with estimates exceeding the number of patients currently seen in cardiology clinics. New developments continue to influence the size of the population of patients with congenital heart disease. Prenatal screening has led to increased rates of termination of pregnancy. Improved management of complications has changed the time and mode of death caused by congenital heart disease. Several genetic and environmental factors have been shown to be involved in the etiology of congenital heart disease, although this knowledge has not yet led to the implementation of preventative measures. In this Review, we give an overview of the etiology, birth prevalence, current prevalence, mortality, and complications of congenital heart disease.

Incidence of congenital heart disease in Qatari children

Objective To study the psychometric properties of a translated version of the Agency for Healthcare Research and Quality Hospital Survey on Patient Safety Culture (HSOPSC) in the Slovenian setting.

Design A cross-sectional psychometric study including principal component and confirmatory factor analysis. The percentage of positive responses for the 12 dimensions (42 items) of patient safety culture and differences at unit and hospital-level were calculated.

Setting Three acute general hospitals.

Participants Census of clinical and non-clinical staff ( n = 976).

Main Outcome Measures Model fit, internal consistency and scale score correlations.

Results Principal component analysis showed a 9-factor model with 39 items would be appropriate for a Slovene sample, but a Satorra-Bentler scaled χ 2 difference test demonstrated that the 12-factor model fitted Slovene data significantly better. Internal consistency was found to be at an acceptable level. Most of the relationships between patient safety culture dimensions were strong to moderate. The relationship between all 12 dimensions and the patient safety grade was negative. The unit-level dimensions of patient safety were perceived better than the dimensions at the hospital-level.

Conclusion The original 12-factor model for the HSOPSC was a good fit for a translated version of the instrument for use in the Slovene setting.

Hospital Survey on Patient Safety Culture in Slovenia: a psychometric evaluation.

A 9-month-old boy and 4-month-old girl presented with severe heart failure. The electrocardiogram showed complete and “incomplete” left bundle branch block, respectively. In both infants noncompaction of the ventricular myocardium was diagnosed with two-dimensional echocardiography. These cases are thought to be the first descriptions of the occurrence of left bundle branch block with noncompaction of the ventricular myocardium. Noncompaction of the ventricular myocardium should be added to the list of the causes for left bundle branch block in children.

BACKGROUND: The most important of the health system inputs, the performance and the benefits the system can deliver, depend largely upon the knowledge, skills, and motivation of those individuals responsible for delivering health services. PURPOSE: This article discusses a study that considered predictors of successful personal involvement in a health care organization. METHODOLOGY/APPROACH: The research included three independent variables (organizational culture, teamwork, and leadership) and one dependent variable (personal involvement). The sample included 734 respondents from 14 Slovene hospitals and three personnel categories: doctors, nurses, and non-health care professionals. FINDINGS: : The results of regression analysis have shown that the dependent variable "personal involvement" can be explained with four independent variables in 49.6% of cases, which are: teamwork (p < .001), level of education (p < .001), transformational leadership (p < .001), and transactional leadership (p = .004). The study has demonstrated that doctors, nurses, and non-health care professionals view their hospitals as having a culture of internal focus, stability, and control. All three groups viewed their level of personal involvement in the organizations as low, whereas nurses and non-health care professionals also expressed a sense of subordination to doctors. PRACTICAL IMPLICATIONS: In addition to other predictors of personal involvement in an organization, which have been researched and confirmed in other studies, our study has also shown the level of education and transactional leadership to be a statistically significant predictor. Managers and leaders must accept responsibility for the existing results regarding personal involvement and start to work on interprofessional collaboration within the organization and outside of it.
