Physician burnout, a work-related syndrome involving emotional exhaustion, depersonalization and a sense of reduced personal accomplishment, is prevalent internationally. Rates of burnout symptoms that have been associated with adverse effects on patients, the healthcare workforce, costs and physician health exceed 50% in studies of both physicians-in-training and practicing physicians. This problem represents a public health crisis with negative impacts on individual physicians, patients and healthcare organizations and systems. Drivers of this epidemic are largely rooted within healthcare organizations and systems and include excessive workloads, inefficient work processes, clerical burdens, work-home conflicts, lack of input or control for physicians with respect to issues affecting their work lives, organizational support structures and leadership culture. Individual physician-level factors also play a role, with higher rates of burnout commonly reported in female and younger physicians. Effective solutions align with these drivers. For example, organizational efforts such as locally developed practice modifications and increased support for clinical work have demonstrated benefits in reducing burnout. Individually focused solutions such as mindfulness-based stress reduction and small-group programmes to promote community, connectedness and meaning have also been shown to be effective. Regardless of the specific approach taken, the problem of physician burnout is best addressed when viewed as a shared responsibility of both healthcare systems and individual physicians. Although our understanding of physician burnout has advanced considerably in recent years, many gaps in our knowledge remain. Longitudinal studies of burnout's effects and the impact of interventions on both burnout and its effects are needed, as are studies of effective solutions implemented in combination. For medicine to fulfil its mission for patients and for public health, all stakeholders in healthcare delivery must work together to develop and implement effective remedies for physician burnout.

Physician burnout: contributors, consequences and solutions

A clutch of '-isms' characterises the approach to consciousness which David Chalmers defends: dualism, epiphenomenalism, functionalism, anti-reductionism, and -probably -panpsychism. (The author would no doubt want 'naturalism' included in the list as well, but as we shall see, Chalmers' predilection to describe his theory as 'scientific' stretches credibility.) While the book does not, as far as I can see, move consciousness research significantly forward, Chalmers succeeds admirably in clarifying the philosophical terrain around and within each of these '-isms' and in questioning the usual assumptions which suggest some of them are mutually exclusive. Because nearly all of what follows is highly critical, I want to be explicit about one thing: I do not think this is a bad book. Throughout, most discussions keep to a very high standard; it's just that they include fatal flaws.

Sexual assault victimization and psychopathology: A review and meta-analysis

Mindfulness‐based meditation practices have received substantial scientific attention in recent years. Mindfulness has been shown to bring many psychological benefits to the individual, but much less is known about whether these benefits extend to others. This meta‐analysis reviewed the link between mindfulness – as both a personality variable and an intervention – and prosocial behaviour. A literature search produced 31 eligible studies (N = 17,241) and 73 effect sizes. Meta‐analyses were conducted using mixed‐effects structural equation models to examine pooled effects and potential moderators of these effects. We found a positive pooled effect between mindfulness and prosocial behaviour for both correlational (d = .73 CI 95% [0.51 to 0.96]) and intervention studies (d = .51 CI 95% [0.37 to 0.66]). For the latter, medium‐sized effects were obtained across varying meditation types and intensities, and across gender and age categories. Preliminary evidence is presented regarding potential mediators of these effects. Although we found that mindfulness is positively related to prosociality, further research is needed to examine the mediators of this link and the contexts in which it is most pronounced.

Does your mindfulness benefit others? A systematic review and meta-analysis of the link between mindfulness and prosocial behaviour

Quality-space theory (QST) explains the nature of the mental qualities distinctive of perceptual states by appeal to their role in perceiving. QST is typically described in terms of the mental qualities that pertain to color. Here we apply QST to the olfactory modalities. Olfaction is in various respects more complex than vision, and so provides a useful test case for QST. To determine whether QST can deal with the challenges olfaction presents, we show how a quality space (QS) could be constructed relying on olfactory perceptible properties and the olfactory mental qualities then defined by appeal to that QS of olfactory perceptible properties. We also consider how to delimit the olfactory QS from other modalities. We further apply QST to the role that experience plays in refining our olfactory discriminative abilities and the occurrence of olfactory mental qualities in non-conscious olfactory states. QST is shown to be fully applicable to and useful for understanding the complex domain of olfaction.

Although vision is the de facto model system of consciousness research, studying olfactory consciousness has its own advantages, as this collection of articles emphatically demonstrates. One advantage of olfaction is its computational and phenomenological simplicity, which facilitates the identification of basic principles. Other researchers study olfactory consciousness not because of its simplicity, but because of its unique features. Together, olfaction's simplicity and its distinctiveness make it an ideal system for testing theories of consciousness. In this research topic, the results of recent research into olfactory consciousness are presented.

Understanding the relation between attention and consciousness is an important part of our understanding of consciousness. Attention, unlike consciousness, can be systematically manipulated in psychophysical experiments and a law-like relation between attention and consciousness is waiting to be discovered. Most attempts to discover the nature of this relation are focused on a special type of attention: spatial visual attention. In this review I want to introduce another type of attention to the discussion: attention to the olfactory modality. I will first clarify the position of attention to smells in a general taxonomy of attention. I will then review the mechanisms and neuroanatomy of attention and consciousness in the olfactory system before using the newly introduced system to provide evidence that attention is necessary for consciousness.
