Treating the Alcoholic: A Developmental Model of Recovery

Today, more than half the world’s population live in cities. By 2030, it is projected that 6 in 10 people will be urban dwellers. Despite numerous planning challenges, cities offer more efficient economies of scale on many levels, including the provision of goods, services and transportation. With sound, risk-informed planning and management, cities can become incubators for innovation and growth and drivers of sustainable development.

The recently reformed Finnish Social Welfare Act obligates public social work to enact structural social work, and to address the communityand structural-level social problems behind individual cases. This article examines how community work and structural social work are conceived by early career social workers. The empirical data consists of 26 project plans by social work Master’s students studying in a working life-based parttime programme. The data includes designs for community work or structural social work to be applied in practice to address a real social problem or issue identified by the students in their working environment. The designs are analysed by content analysis and related to the theoretical categorizations of community strategies by Rothman, the community work models by Popple, and the task fields of structural social work by Pohjola. However, the strong evidence of mixed models in the results allows to suggest that categorizations as theoretical-conceptual frames are rather useful for identifying rich diversities and combinations, instead of clear-cut categories of community work and structural social work. The designs reflect the challenges created by the managerialist service systems themselves. They seek more cooperative structures to meet the reallife needs of local communities. It can be concluded that structural social work can become a powerful approach when taken as a legal mandate in the hands of early career social workers, enacting community and political perspectives in Finnish social work at all levels. This requires that they will not be prohibited or tamed in its applications.

Next-generation modelling of community work and structural social work in Finland

Individuals residing in more socioeconomically disadvantaged neighborhoods experience greater uncertainty through insecurity of basic needs such as food, employment, and housing, compared with more advantaged neighborhoods. Although the neurobiology of uncertainty has been less frequently examined in relation to neighborhood disadvantage, there is evidence that neighborhood disadvantage is associated with widespread neural alterations.Recently traumatically injured participants (n = 90) completed a picture anticipation task in the magnetic resonance imaging scanner, in which they viewed images presented in a temporally predictable or unpredictable manner. We investigated how neighborhood disadvantage (via area deprivation index [ADI]) was related to neural activation during anticipation and presentation of negative and neutral images after accounting for individual factors (i.e., age, gender, income, acute posttraumatic stress symptoms).There was a significant interaction during the anticipation period such that higher ADI rankings were related to greater activation of the right anterior cingulate cortex to predictable versus unpredictable neutral stimuli. Although no other robust interactions emerged related to ADI, we note several novel simple effects of ADI during anticipation and presentation periods in the hippocampus and prefrontal, cingulate, and occipital cortices.Together, these results may represent an adaptive response to predictable and/or negative stimuli, stemming from chronic exposure to socioeconomic-based uncertainties. Although effects were modest, future work should continue to examine pretrauma context on posttrauma outcomes. To better understand trauma outcomes, it is imperative that researchers consider the broader context in which trauma survivors reside.

Neighborhood Socioeconomic Disadvantage and the Neurobiology of Uncertainty in Traumatically Injured Adults

In the CAT model it is important to include the person with an addiction and close family members. In this quantitative research we compared these two perspectives to detect the differences in their views on the functioning of the support group. Research was conducted using stratified random sampling in 40 CATs in Croatia with 653 participants: 453 members with an addiction and 200 supporting or accompanying family members. Research instruments for assessing different aspects of CAT as a support group were developed by the authors. Data were analysed using MANOVA analysis to detect differences in the two perspectives. The results show that both groups demonstrate a significant level of satisfaction with different aspects of group work. However, MANOVA indicated the following differences: the accompanying family members are usually women, accompanying family members attend meetings less frequently and have lower insight into the professional’s work, group relations and CAT’s contribution to their personal development. The research is one of the rare empirical insights into CAT’s functioning as a treatment group, with an attempt to distinguish heterogeneity in a membership. Since family members are somewhat more critical towards some features of CATs, in the future more emphasis should be placed on their active engagement in the group’s processes.

How do Clubs of Alcoholics in Treatment Function? Differences Between the Perspectives of Members with Addiction and Family Members

A distinctive form of posttreatment support for persons in recovery from alcoholism (alcohol use disorders) is the club of treated alcoholics (CTA). These groups are in the tradition of Alcoholics Anonymous with several important distinctions: the CTA groups are led by a health or social care professional, family members are included in the group, and groups are encouraged to be active in local community. This is a descriptive study of 40 randomly selected CTA groups with a focus on how participation in these groups improves the level of psychosocial functioning in the participants with a focus on how the participants relate to each other and the assigned health care professional.

Based on theory of neighbourhood effect and extensive research literature, it is obvious that deprived communities have clear impact on individual well-being. Residents face poorly developed infrastructure, greater ecological risks, less safety and higher rates of violence and crime along with greater occurrence of juvenile delinquent behaviour. Inhabitants will more often encounter poverty, unemployment and poorer residential conditions. There are higher mortality rates, lower levels of functional abilities, poorer health indicators in the development of children and specifically in some groups a higher prevalence of diseases such as cardiovascular and respiratory diseases. Poorer health indicators are linked to lower levels of personal health awareness and a higher presence of health-risk behaviour. Results also reveal poorer family relationships and mental health problems. Inhabitants in disadvantaged communities have a less healthy lifestyle, including inadequate diet and less physical activity. Limiting effects contribute to reduced educational opportunities, and the growth rate in particular segments is slower than in developed communities leading to an increased inequality gap. However, the link between community deprivation and individual well-being is far from being linear. This chapter will reveal many complex dimensions of this relationship that are of special value for social work practice in deprived communities.
