Qualitative systematic review of the acceptability, feasibility, barriers, facilitators and perceived utility of using physical activity in the reduction of and abstinence from alcohol and other drug use

Abstract Aims To provide an overview of research literature on ageing and older people who use illicit opioids and stimulants by documenting the conceptual frameworks used and content areas that have been investigated. Methods We conducted a scoping review of literature relating to ageing and older people who use illicit stimulants and opioids, defining ‘older’ as 40 years and above. Primary studies, secondary studies and editorials were included. Searches were conducted in PubMed and Embase in July 2020 and March 2021; the Cochrane library was searched in November 2021. Charted data included methodological details, any conceptual frameworks explicitly applied by authors and the content areas that were the focus of the publication. We developed a hierarchy of content areas and mapped this to provide a visual guide to the research area. Results Of the 164 publications included in this review, only 16 explicitly applied a conceptual framework. Seven core content areas were identified, with most publications contributing to multiple content areas: acknowledgement of drug use among older people (n = 64), health status (n = 129), health services (n = 109), drug use practices and patterns (n = 84), social environments (n = 74), the criminal legal system (n = 28) and quality of life (n = 15). Conclusions The literature regarding older people who use illicit drugs remains under‐theorized. Conceptual frameworks are rarely applied and few have been purposely adapted to this population. Health status and health services access and use are among the most frequently researched topics in this area.

Ageing and older people who use illicit opioids, cocaine or methamphetamine: a scoping review and literature map

Introduction : Au Royaume-Uni, des systemes d’orientation a l’activite physique (SOAP) ont ete developpes pour la population generale. Les objectifs de cette etude etaient d’evaluer l’efficacite de ces SOAP dans les domaines de la sante et sur le plan medico-economique ainsi que d’identifier les facteurs associes a l’orientation, l’adhesion et la reussite d’un SOAP.Methodes : Les etudes etaient selectionnees parmi les bases de donnees Medline, Pascal et Cochrane. Les termes de recherche « exercise referral schemes » ou « exercise referral systems » ont ete utilises. Les etudes incluses etaient les suivantes : essais randomises controles, essais controles, meta-analyse, etudes qualitatives.Resultats : Au total, 26 articles ont ete inclus. Les SOAP entrainent une augmentation significative du nombre de participants realisant 90 a 150 minutes d’activite physique moderee hebdomadaire versus soins usuels. En revanche, les analyses realisees sur les differents marqueurs de sante (i.e. qualite de vie, depression, indice de masse corporelle, taux d’hemoglobine glyquee, anxiete) ne retrouvent aucune difference en faveur des SOAP a long terme. Les SOAP semblent plus particulierement cout-efficaces chez les sujets a risque ou touches par une maladie chronique. L’adhesion aux interventions semble jouer un role cle sur l’efficacite des SOAP.Discussion : Les SOAP semblent avoir des resultats mitiges en termes de benefices pour la sante. L’analyse des donnees probantes pourrait amener une mise en place des SOAP plus efficiente et adaptee en France.

The effectiveness and lessons learned from UK exercise referral systems

Objectives To test whether older drug users (aged 40 and over) could be recruited to an exercise referral (ER) scheme, to evaluate the feasibility and acceptability and measure the impact of participation on health. Design Observational pilot. Setting Liverpool, UK. Participants (1) 12 men and 5 women recruited to ER. (2) 7 specialist gym instructors. Outcome measures Logistic feasibility and acceptability of ER and associated research, rate of recruitment, level of participation over 8 weeks and changes in health. Results 22 gym inductions were arranged (recruitment time: 5 weeks), 17 inductions were completed and 14 participants began exercising. Attendance at the gym fluctuated with people missing weeks then re-engaging; in week 8, seven participants were in contact with the project and five of these attended the gym. Illness and caring responsibilities affected participation. Participants and gym instructors found the intervention and associated research processes acceptable. In general, participants enjoyed exercising and felt fitter, but would have welcomed more support and the offer of a wider range of activities. Non-significant reductions in blood pressure and heart rate and improvements in metabolic equivalents (METs; a measure of fitness) and general well-being were observed for eight participants who completed baseline and follow-up assessments. The number of weeks of gym attendance was significantly associated with a positive change in METs. Conclusions It is feasible to recruit older drug users into a gym-based ER scheme, but multiple health and social challenges affect their ability to participate regularly. The observed changes in health measures, particularly the association between improvements in METs and attendance, suggest further investigation of ER for older drug users is worthwhile. Measures to improve the intervention and its evaluation include: better screening, refined inclusion/exclusion criteria, broader monitoring of physical activity levels, closer tailored support, more flexible exercise options and the use of incentives.
