Key findings  More terbutaline was entrained on the inhalation filter in position A than in position B (P < 0001) for both nebulisers These amounts were greater (P < 0001) for AERO than SIDE due to a smaller (P < 0001) residual volume The mean (SD) fine particle doses for AEROA, AEROB, SIDEA and SIDEB were 131 (02), 113 (014), 056 (003) and 039 (013) mg These amounts from AEROA were significantly greater (P < 0001) than those of the other three methods

Conclusions  The results highlight the differences between nebulisers and the influence on the placement of the nebuliser in the NIV circuit

In-vitro characterisation of the nebulised dose during non-invasive ventilation

Size distributions of droplets generated by nebulizers are difficult to determine because of evaporation after aerosolization. We describe a method whereby a Next Generation Pharmaceutical Impactor (NGI; MSP Corporation, Shoreview, MN) is refrigerated at 5°C before connecting it to the nebulizer in order to ensure an environment inside the NGI at close to 100% relative humidity (RH). This, in turn, reduces droplet evaporation between the nebulizer and impaction. The method development was performed with a Pari LC Plus jet nebulizer operated at 2.0 bar, with the NGI set at a flow rate of 15 L/min and with salbutamol 5.0 mg/mL as the test solution. The droplet size distributions were expressed in terms of mass median aerodynamic diameter (MMAD) and geometric standard deviation (GSD). The pulmonary route has been used with success for the treatment of both lung (asthma) and systemic diseases (diabetes). The fate of an inhaled drug (absorption and deposition) within human lungs has great importance, particularly in drug development and quality control. This article focuses on the various methods that are now applied for aerosol fate investigation. Several assessment methods, ranging from in vitro assays (impaction and optical systems) to in vivo experiments (imaging and pharmacological methods), are described. In vitro assays measure particle size distribution and emitted drug dose, which could be predictive of lung deposition pattern in vivo. Assessment methods of inhaled aerosols: technical aspects and applications.

Characterization of the aerosolized dose emitted from a nebulized system can be determined using CEN (prEN13544-1) methodology and more recently with a Next Generation Impactor (NGI), but evaporative effects can influence the results. We have investigated these characteristics using different flows and cooling with the NGI and compared the results to the standard CEN method using two different nebulizer systems. The NGI was operated using flows of 15 and 30 L min−1 at room (ROOM) temperature and immediately after cooling at 5°C for 90 min (COLD). Two nebulizer systems, the Sidestream jet nebulizer (SIDE) and the Aeroneb Pro (AERO), were used to nebulize terbutaline sulphate respiratory solution. The CEN method was also used to provide the aerodynamic characteristics of the aerosolized dose from these two nebulizer systems. Aerodynamic characteristics of nebulized terbutaline sulphate using the Next Generation Impactor (NGI) and CEN method.

Different approaches have been employed for in vitro assessment of the aerosol particle size generated by inhalation devices. In this study, aerosols from the Omron MicroAir vibrating mesh (VM) nebulizer were measured by cascade impaction (CI) using the MSP Next Generation Pharmaceutical Impactor (NGI), the ThermoAndersen Cascade Impactor (ACI), and by time-of-flight (TOF) analysis with the TSI 3321 Aerodynamic Particle Sizer Spectrometer (APS). The VM nebulizer was evaluated with sodium fluoride (NaF; 2.5%) and with generic albuterol (0.083%). Aerosol particle size (MMAD), respirable fractions (RF 0.05). TOF analysis by APS with albuterol gave significantly larger calculated MMAD (cMMAD) than either CI method (7.2 microm; p < 0.001). TOF measurements of nebulized albuterol at RT and 4 degrees C were equivalent. In summary, the results of VM nebulized NaF and albuterol were more consistent and generally equivalent when determined by NGI (at RT and 4 degrees C) and ACI analysis (at 4 degrees C). In contrast, aerosol particle sizes measured by TOF in the APS at both RT and 4 degrees C were larger than results obtained by CI. Differences in aerosol particle distribution obtained by different analysis methods should be considered while evaluating the in vitro performance of VM nebulizers.

Comparative analysis of methods to measure aerosols generated by a vibrating mesh nebulizer.

BACKGROUND: During operation of a jet nebulizer, the temperature of the nebulizer outlet could decrease by more than 10°C, depending on the nebulizer type and operating conditions, such as driving flow rate and fill volume. The droplet size distribution generated from the nebulizer can be measured by a cascade impactor. However, when the cascade impactor is operated at ambient room temperature, the droplets could evaporate because of the temperature difference between the nebulizer outlet and the body of the impactor. METHODS: An 8-stage cascade impactor was used to measure the particle size distribution from 4 different types of jet nebulizer (LC Plus, Side-Stream, VixOne, and Micromist) in 2 temperature conditions: ambient (22°C) and low (10°C).
