Fur asylsuchende Kinder und Jugendliche besteht ein erhohtes Risiko der Ansteckung mit Tuberkulose (TB) aufgrund ihrer Herkunft sowie durch Exposition wahrend der Flucht und beim Aufenthalt in Gemeinschaftsunterkunften. Gemas Infektionsschutzgesetz haben asylsuchende Personen in Gemeinschaftsunterkunften ein arztliches Zeugnis daruber vorzulegen, dass bei ihnen keine Anhaltspunkte fur das Vorliegen einer ansteckungsfahigen Lungen-TB vorhanden sind. Symptom- und/oder thoraxrontgenbasiertes TB-Screening haben bei Kindern und Jugendlichen im Vergleich zu Erwachsenen eine geringere Sensitivitat und Spezifitat. Zudem soll in dieser Altersgruppe ein besonders restriktiver Umgang mit ionisierender Strahlung gewahlt werden. Deshalb wird ein immunodiagnostisches TB-Screening mittels Tuberkulinhauttest (THT) oder „interferon-gamma release assay“ (IGRA) empfohlen. Der THT ist die Methode der Wahl bei Kindern < 5 Jahren; zwischen 5 und 14 Jahren kann ein THT oder IGRA verwendet werden. Dies soll bei allen asylsuchenden Kindern und Jugendlichen < 15 Jahren unabhangig von der TB-Inzidenz des Herkunftslands durchgefuhrt werden. Bei positivem Testergebnis sollen weitere Abklarung und Therapie gemas bestehender nationaler Empfehlung erfolgen.

It is often said that pulmonary tuberculosis is not common in children. It is true that the number of cases admitted to a children's hospital bearing that diagnosis is comparatively small, and that the number of deaths registered as from that cause forms but a small proportion of the total deaths of children from tuberculosis of all types. But it must be recognised that consideration only of clinically manifest pulmonary tuberculosis gives a wholly false and misleading conception of the real importance and frequency of tuberculous lung lesions in children. Vast numbers of cases with primary lesions in the lungs never declare themselves as pulmonary tuberculosis. Patients with inflammatory bowel disease are predisposed to opportunistic infections. We report 2 cases of disseminated tuberculosis in adolescents receiving a TNF antagonist, infliximab. Both had negative baseline tuberculin skin tests. Multimodal testing using tuberculin skin tests and interferon gamma release assays at the time of inflammatory bowel disease diagnosis and annually may increase the sensitivity of LTBI testing in these high risk children.

In the whole world, tuberculosis (TB) continues being an important health problem. It produces a great number of deaths in children, mainly in the wake of the HIV epidemic. In addition, the globalization with the increased of the international travels and the immigration makes that TB control be more difficult. Clinical, Radiological and Inmunological Features in Children with Pulmonary Tuberculosis: A Review

Objective Recent studies found that a chest x-ray (CXR) has limited value in the assessment of asymptomatic adults with tuberculosis (TB) infection. We aimed to determine in asymptomatic children with a positive tuberculin skin test and/or interferon-γ release assay (TST/IGRA) whether a CXR identifies findings suggestive of pulmonary TB. Design, setting and patients All children with TB infection (defined as TST ≥10 mm and/or positive IGRA) presenting to The Royal Children's Hospital Melbourne during a 54-month period were included. All CXRs were reviewed by a senior radiologist blinded to the clinical details. The medical records of those with radiological abnormalities suggestive of TB were examined to identify those who were asymptomatic when the CXR was done. Demographical data were also collected. Results CXRs were available for 268 of 330 TB-infected children, of whom 60 had CXR findings suggestive of TB. Of the 57 for whom clinical details were available, 26 were asymptomatic. Of these asymptomatic children with radiological abnormalities suggestive of TB, 6 had CXR findings suggestive of active TB, 14 had CXR findings suggestive of prior TB and 6 had isolated non-calcified hilar lymphadenopathy. The six with findings suggestive of active TB represented 2.6% (95% CI 0.9 to 5.5%) of asymptomatic TST/IGRA-positive children with evaluable CXRs. One child with isolated hilar lymphadenopathy had microbiologically-confirmed TB. Conclusions In contrast to the results from studies in adults, a CXR identified a small but noteworthy number of children with findings suggestive of pulmonary TB in the absence of clinical symptoms.
