Infections pose a significant threat to morbidity and mortality during treatments for pediatric cancer patients. Efforts to minimize the risk of infection necessitate preventive measures encompassing both environmental and host-focused strategies. While a substantial number of infections in oncologic patients originate from microorganisms within their native microbiological environment, such as the oral cavity, intestines, and skin, the concrete risk of bloodstream infections linked to the consumption of contaminated food and beverages in the community cannot be overlooked. Ensuring food quality and hygiene is essential to mitigating the impact of foodborne illnesses on vulnerable patients. The neutropenic diet (ND) has been proposed to minimize the risk of sepsis during neutropenic periods. The ND aims to minimize bacterial entry into the gut and bacterial translocation. However, a standardized definition for ND and consensus guidelines for specific food exclusions are lacking. Most centers adopt ND during neutropenic phases, but challenges in achieving caloric intake are common. The ND has not demonstrated any associated benefits and does not ensure improved overall survival. Consequently, providing unified and standardized food safety instructions is imperative for pediatric patients undergoing hematopoietic cell transplantation (HCT). Despite the lack of evidence, ND is still widely administered to both pediatric and adult patients as a precautionary measure. This narrative review focuses on the impact of foodborne infections in pediatric cancer patients and the role of the ND in comparison to food safety practices in patients undergoing chemotherapy or HCT. Prioritizing education regarding proper food storage, preparation, and cooking techniques proves more advantageous than merely focusing on dietary limitations. The absence of standardized guidelines underscores the necessity for further research in this field.

Food Safety and Pediatric Cancer: Neutropenia is a common complication of chemotherapy, which poses a high risk of infection and mortality. Neutropenic diet has historically been recommended for those undergoing chemotherapy. The rationale is to reduce the risk of foodborne infection by avoiding foods considered to be of high microbial risk. However, evidence for this diet is limited, and there is a lack of national consensus guidelines. Aim: Ascertain food safety advice across specialist centers providing high-dose chemotherapy for malignancy or stem cell transplants in the United Kingdom. Materials and Methods: Dietitians at 22 centers were contacted to complete a questionnaire regarding food safety guidance implemented at their center for pediatric patients undergoing high-dose chemotherapy or stem cell transplants. Questions related to restricted foods, specific guidelines implemented, ward food provision, and timings of food provision. Results: Sixteen centers responded (73%). Many aspects of neutropenic diet were consistent across centers; avoidance of unpasteurized dairy products (94%), raw/undercooked meat (94%), and unpasteurized pâté (88%). There was a lack of consistency regarding water sources used on wards and unpeeled fruits and vegetables. Conclusions: Food safety guidance for neutropenic patients differs across centers, with some practices seeming outdated and nonevidence based. A national review of food safety guidance should be considered to provide a standardized approach.

A Cross-sectional Survey to Review Food Safety Practices Within Pediatric Oncology and Stem Cell Transplant Centers in the United Kingdom

<h3>Background &amp; Aim</h3> Paediatric cardiomyopathy causes heart failure which may require transplantation; 30–50% of patients listed for transplant require a bridge strategy such as a VAD. Whilst awaiting transplant. VAD patients have a significant mortality risk partly due to adverse events and worsening heart failure symptoms. This review assesses feeding practices for VAD patients within a UK paediatric hospital. <h3>Methods</h3> A single-centre chart review for seventeen patients admitted for VAD-implantation between May 2019-April 2022. Data included: anthropometrics, feed type, feeding routes at 6-time points; VAD-implantation, months 2,4,6,9 and immediately pre-transplant. <h3>Results</h3> The mean age of VAD-implantation was 4.5 years (±3.1SD). Median time to transplant was 215 days (interquartile range 83,272). Weight-for-age z-score (WAZ) improved from -1.62 at VAD-implantation (SD±0.83) to -0.71 at transplant (SD±1.08) (p=0.001). Commencing enteral (EN) or parenteral nutrition (PN) post VAD-implantation takes mean time of 2 days (±SD1.18). From VAD-implantation to transplant, PN usage decreased from 41% to 21%; whole protein EN increased from 18% to 29%; partially hydrolysed EN increased from 29% to 50% and amino acid EN decreased from 12% to 0%. Oral intake contributed to nutritional intake for 33% of patients at months 2 and 4, 11% at 6 months and 50% at 9 months. Gastric EN contributed to nutritional intake at 2 months for 60%, 4 months for 50%, 6 months for 56% and 9 months for 67% of patients. Post-pyloric EN contributed to intake in 33% at months 2, 6 and 9 and peaked at 50% at 4 months. No patients were exclusively orally fed. <h3>Conclusion</h3> Whilst awaiting transplant, growth improved post VAD-implantation. Feeding routes and types change throughout the VAD course and oral intake is poor with most patients relying on EN. Gastric feeding using hydrolysed formula is the best tolerated option however each patient’s journey may differ.

<h3>Background</h3> The use of ventricular assist devices (VADs) in the management of children in heart failure is increasing, with VADs being used as bridges to transplant. Nutrition plays a critical role in improving outcomes post-VAD insertion and post-transplant, including reducing the risk of infections and mortality. Children in heart failure are often malnourished due to early satiety and poor enteral tolerance. Methods such as post-pyloric feeding and use of hydrolysed feeds can improve enteral intolerance in other paediatric groups. However, in the United Kingdom, there are no clinical guidelines or recommendations regarding feeding routes or feed types for children who are supported by VADs. <h3>Aims</h3> To review the evidence regarding feeding route and feed type used in paediatric patients using VADs. <h3>Methods</h3> The PRISMA method for systematic reviews was followed. Inclusion criteria included: papers dated between 2002-2022, paediatric population, reported on feeding during VAD therapy. Exclusion criteria included: papers dated prior to 2002, adult population, no documentation of feeding type or route. The pubmed database was searched to identify relevant papers. <h3>Results</h3> One hundred and twenty papers were identified, six articles met eligibility criteria and were included in the review. Papers were excluded if they did not meet inclusion criteria, were unable to access, duplicated or not relevant upon screening. One paper reported patients established on jejunal feeding and recommended this route; one paper reported patients established on gastric feeding; one paper reported patients orally eating and one paper recommended orally feeding as soon as the patient was stable. Two papers reported patients on both enteral and parenteral nutrition. No papers documented or recommended types of feed. <h3>Conclusion</h3> There is a paucity of information regarding feeding routes and feed types for paediatric patients with VADs. Well-designed interventional research is required to optimise the nutritional management of this complex population.
