Large language models (LLMs) like ChatGPT appear capable of performing a variety of tasks, including answering patient eye care questions, but have not yet been evaluated in direct comparison with ophthalmologists. It remains unclear whether LLM-generated advice is accurate, appropriate, and safe for eye patients.To evaluate the quality of ophthalmology advice generated by an LLM chatbot in comparison with ophthalmologist-written advice.This cross-sectional study used deidentified data from an online medical forum, in which patient questions received responses written by American Academy of Ophthalmology (AAO)-affiliated ophthalmologists. A masked panel of 8 board-certified ophthalmologists were asked to distinguish between answers generated by the ChatGPT chatbot and human answers. Posts were dated between 2007 and 2016; data were accessed January 2023 and analysis was performed between March and May 2023.Identification of chatbot and human answers on a 4-point scale (likely or definitely artificial intelligence [AI] vs likely or definitely human) and evaluation of responses for presence of incorrect information, alignment with perceived consensus in the medical community, likelihood to cause harm, and extent of harm.A total of 200 pairs of user questions and answers by AAO-affiliated ophthalmologists were evaluated. The mean (SD) accuracy for distinguishing between AI and human responses was 61.3% (9.7%). Of 800 evaluations of chatbot-written answers, 168 answers (21.0%) were marked as human-written, while 517 of 800 human-written answers (64.6%) were marked as AI-written. Compared with human answers, chatbot answers were more frequently rated as probably or definitely written by AI (prevalence ratio [PR], 1.72; 95% CI, 1.52-1.93). The likelihood of chatbot answers containing incorrect or inappropriate material was comparable with human answers (PR, 0.92; 95% CI, 0.77-1.10), and did not differ from human answers in terms of likelihood of harm (PR, 0.84; 95% CI, 0.67-1.07) nor extent of harm (PR, 0.99; 95% CI, 0.80-1.22).In this cross-sectional study of human-written and AI-generated responses to 200 eye care questions from an online advice forum, a chatbot appeared capable of responding to long user-written eye health posts and largely generated appropriate responses that did not differ significantly from ophthalmologist-written responses in terms of incorrect information, likelihood of harm, extent of harm, or deviation from ophthalmologist community standards. Additional research is needed to assess patient attitudes toward LLM-augmented ophthalmologists vs fully autonomous AI content generation, to evaluate clarity and acceptability of LLM-generated answers from the patient perspective, to test the performance of LLMs in a greater variety of clinical contexts, and to determine an optimal manner of utilizing LLMs that is ethical and minimizes harm. 

Comparison of Ophthalmologist and Large Language Model Chatbot Responses to Online Patient Eye Care Questions

In this paper, we introduce a unified and generalist Biomedical Generative Pre-trained Transformer (BiomedGPT) model, which leverages self-supervision on large and diverse datasets to accept multi-modal inputs and perform a range of downstream tasks. Our experiments demonstrate that BiomedGPT delivers expansive and inclusive representations of biomedical data, outperforming the majority of preceding state-of-the-art models across five distinct tasks with 20 public datasets spanning over 15 unique biomedical modalities. Through the ablation study, we also showcase the efficacy of our multi-modal and multi-task pretraining approach in transferring knowledge to previously unseen data. Overall, our work presents a significant step forward in developing unified and generalist models for biomedicine, with far-reaching implications for improving healthcare outcomes.

BiomedGPT: A Unified and Generalist Biomedical Generative Pre-trained
  Transformer for Vision, Language, and Multimodal Tasks

The remarkable success of deep learning has prompted interest in its application to medical imaging diagnosis. Even though state-of-the-art deep learning models have achieved human-level accuracy on the classification of different types of medical data, these models are hardly adopted in clinical workflows, mainly due to their lack of interpretability. The black-box-ness of deep learning models has raised the need for devising strategies to explain the decision process of these models, leading to the creation of the topic of eXplainable Artificial Intelligence (XAI). In this context, we provide a thorough survey of XAI applied to medical imaging diagnosis, including visual, textual, example-based and concept-based explanation methods. Moreover, this work reviews the existing medical imaging datasets and the existing metrics for evaluating the quality of the explanations. In addition, we include a performance comparison among a set of report generation-based methods. Finally, the major challenges in applying XAI to medical imaging and the future research directions on the topic are also discussed.

Explainable Deep Learning Methods in Medical Image Classification: A Survey

Recent advances in the field of large language models (LLMs) underline their high potential for applications in a variety of sectors. Their use in healthcare, in particular, holds out promising prospects for improving medical practices. As we highlight in this paper, LLMs have demonstrated remarkable capabilities in language understanding and generation that could indeed be put to good use in the medical field. We also present the main architectures of these models, such as GPT, Bloom, or LLaMA, composed of billions of parameters. We then examine recent trends in the medical datasets used to train these models. We classify them according to different criteria, such as size, source, or subject (patient records, scientific articles, etc.). We mention that LLMs could help improve patient care, accelerate medical research, and optimize the efficiency of healthcare systems such as assisted diagnosis. We also highlight several technical and ethical issues that need to be resolved before LLMs can be used extensively in the medical field. Consequently, we propose a discussion of the capabilities offered by new generations of linguistic models and their limitations when deployed in a domain such as healthcare.

Recent Advances in Large Language Models for Healthcare

The proposed model for automatic clinical image caption generation combines the analysis of radiological scans with structured patient information from the textual records. It uses two language models, the Show-Attend-Tell and the GPT-3, to generate comprehensive and descriptive radiology records. The generated textual summary contains essential information about pathologies found, their location, along with the 2D heatmaps that localize each pathology on the scans. The model has been tested on two medical datasets, the Open-I, MIMIC-CXR, and the general-purpose MS-COCO, and the results measured with natural language assessment metrics demonstrated its efficient applicability to chest X-ray image captioning.
