The present, updated document describes the fourth iteration of recommendations for the hepatic use of contrast-enhanced ultrasound, first initiated in 2004 by the European Federation of Societies for Ultrasound in Medicine and Biology. The previous updated editions of the guidelines reflected changes in the available contrast agents and updated the guidelines not only for hepatic but also for non-hepatic applications. The 2012 guideline requires updating as, previously, the differences in the contrast agents were not precisely described and the differences in contrast phases as well as handling were not clearly indicated. In addition, more evidence has been published for all contrast agents. The update also reflects the most recent developments in contrast agents, including U.S. Food and Drug Administration approval and the extensive Asian experience, to produce a truly international perspective. These guidelines and recommendations provide general advice on the use of ultrasound contrast agents (UCAs) and are intended to create standard protocols for the use and administration of UCAs in liver applications on an international basis to improve the management of patients.

The introduction of ultrasound contrast agents has rendered contrast-enhanced ultrasound (CEUS) a valuable complementary technique to address clinically significant problems. This pictorial review describes the use of CEUS guidance in abdominal intervention and illustrates such application for a range of clinical indications. Clinical application of CEUS discussed include commonly performed abdominal interventional procedures, such as biopsy, drainage, nephrostomy, biliary intervention, abdominal tumor ablation and its subsequent monitoring, and imaging of vascular complications following abdominal intervention. The purpose of this article is to further familiarize readers with the application of CEUS, particularly its specific strength over alternative imaging modalities, in abdominal intervention.

Seit Dezember 2019 verbreitet sich das neuartige Coronavirus SARS-CoV-2 (Severe Acute Respiratory Syndrome – Corona Virus-2) rasch im Sinne einer weltweiten Pandemie. Dies stellt Kliniker und Krankenhauser vor grose Herausforderungen und belastet die Gesundheitssysteme vieler Lander in einem nie dagewesenen Ausmas. Die Mehrheit der Patienten mit Coronavirus Disease 2019 (COVID-19) zeigt lediglich milde Symptome wie Husten und Fieber. Allerdings benotigen etwa 8 % eine stationare Behandlung. Der fruhzeitigen Klarung, ob eine stationare und ggfs. intensivmedizinische Behandlung medizinisch sinnvoll und vom Patienten gewollt ist, kommt in der Pandemie eine besondere Bedeutung zu. Die akute hypoxamische respiratorische Insuffizienz mit Dyspnoe und hoher Atemfrequenz (> 30/min) fuhrt i. d. R. zur Aufnahme auf die Intensivstation. Oft finden sich dann bereits bilaterale pulmonale Infiltrate/Konsolidierungen oder auch Lungenembolien in der Bildgebung. Im weiteren Verlauf entwickeln einige dieser Patienten ein akutes Lungenversagen (Acute Respiratory Distress Syndrome; ARDS). Eine Sterblichkeitsreduktion einer verfugbaren medikamentosen Therapie bei schwerer COVID-19-Erkrankung ist bisher lediglich fur Dexamethason in randomisiert, kontrollierten Studien nachgewiesen. Das Hauptziel der supportiven Therapie besteht in der Sicherstellung einer ausreichenden Oxygenierung. Die invasive Beatmung und wiederholte Bauchlagerung sind dabei wichtige Elemente in der Behandlung von schwer hypoxamischen COVID-19-Patienten. Die strikte Einhaltung der Basishygiene, einschlieslich der Handehygiene, sowie das korrekte Tragen von adaquater personlicher Schutzausrustung sind im Umgang mit den Patienten unabdingbar. Medizinisch notwendige Handlungen am Patienten, die zur Aerosolbildung fuhren konnten, sollten mit auserster Sorgfalt und Vorbereitung durchgefuhrt werden.

Die Lungen- und Thoraxsonographie ist neben der Computertomographie und der Labordiagnostik eine weitere Untersuchungsmodalitat bei der COVID-19-Erkrankung. Sie eignet sich zur Erweiterung der klinisch-korperlichen Untersuchung, weil sie sensitiv die Lungenoberflache untersuchen kann. Diese verandert sich in den Sonogrammen mit bislang haufig beschriebenen Mustern bei Patienten mit COVID-19-Pneumonie und im Krankheitsverlauf. Deutsche Fachgesellschaften der klinischen Akut‑, Notfall- und Intensivmedizin sowie der Bildgebung, die mit der Versorgung von Patienten mit SARS-CoV-2-Infektion und der COVID-19-Erkrankung befasst sind, haben unter der Fuhrung der Deutschen Gesellschaft Interdisziplinare Notfall- und Akutmedizin e. V. (DGINA) Empfehlungen zur Lungen- und Thoraxsonographie abgestimmt. Das Dokument wurde uber einen transparenten Prozess durch eine Expertengruppe und Delegierte der Fachgesellschaften erstellt. Quellen der ersten internationalen 200 Fallanalysen wurden zusammengefasst. Bislang haufig beschriebene thoraxsonographische Muster werden vorgestellt. Anhand von Fallbeispielen und multimedialen Inhalten soll das Dokument die Anwender unterstutzen und Qualitatsmerkmale und das Potenzial der Thorax- und Lungensonographie aufzeigen. Die Deutsche Gesellschaft fur Ultraschall in der Medizin e. V. (DEGUM) fuhrt dazu eine Multizenterstudie durch (Studienkoordination TU Munchen).

Endocavitary use of contrast agents in sonography (US) is a relatively new method in diagnostic imaging, competing against gray-scale US, fluoroscopy, and endoscopy. This article describes established indications, demonstrates the techniques of evolving applications, and discusses their potential benefits. These benefits include the ability to obtain precise information about the placement of drains and the extent of fluid collections, and to accurately identify the location and features of strictures in various organs, and those of complications of fluid collections or abscesses, without resorting to ionizing radiation. © 2014 Wiley Periodicals, Inc.
