Research sciences and medical societies have recently shifted into using cost-effective biosensors to test food & water contaminants, control human biologic processes, assess precise health diagnosis, and more. Researchers and medical practitioners need safe and cheaper means of performing their research, ensuring public safety, and delivering customised health options to patients. One such solution can be easily carried out by using biosensors. In the new medical field, biomedical studies of diagnosis are of growing significance. Biosensors' applications are for screening infectious to early detection, chronic disease treatment, health management, and well-being surveillance. Improved biosensors technology qualities allow the ability to detect disease and track the body's response to care. Sensor technology is integral to numerous, low-cost, and improved-form factors feasible in modern medical devices. Biosensors have good potential, as it is easy, scalable and effective in manufacturing processes. This paper discusses biosensors and their significant benefits in the medical field. Distinctive capabilities of biosensors in healthcare services and for cardiovascular disease are provided and shown diagrammatically. The paper also discusses various diagnostic biosensors for cardiovascular diseases and provides novel aspects of biosensors for clinical and allied services. Thereby paper provides significant advancements in biosensors in the medical field. Finally, fourteen major applications of biosensors in the medical field are identified and discussed. Biosensors' intelligent wearable properties now allow older people to control their health with lesser interference, and it directly exchanges their medical-related information with healthcare providers, thereby reducing hospital visits. Thus, biosensors have countless prospects for consumer and commercial uses in wellness, fitness, athletics, etc. Linked biomedical devices, apps, firmware, and sophisticated algorithms will do a lot, including allowing major new medical therapies and informing users about health reform, providing solutions and advice informed by real-time evidence.

Biosensors applications in medical field: A brief review

Robust, reliable, and quantitative detection of biomarkers at ultra-low concentration is of great importance in clinical settings. Biosensor, an analytical device used for sensitive and selective detection of biomarkers offers various advantages over the conventional clinical diagnosis, which is both time consuming and not suitable for point of care/onsite diagnosis. A revolution in the understanding and synthesis of nanomaterials in the last couple of decades contributed significantly to the development of the biosensors in terms of sensitivity, catalytic activity, biocompatibility, and robustness. Additionally, nanomaterials help in miniaturization of the sensing platform and helping in the commercial success of portable biosensor kits. Surface engineering equally contributed to the biosensor development by ensuring a reproducible and stable sensing surface, efficient analyte-biorecognition element interaction, and reduced fouling effect in biological solution. Due to nanomaterial integration and surface engineering, biosensors are now equally sensitive to the lab-based sophisticated instruments to detect a wide range of molecules of clinical significance. In this review, various types of biosensors, their designs, and their working principles have been discussed. A detailed account of various types of nanomaterials, their functionalization and characterization have also been discussed. The analytical performances of biosensors for both clinical validation and analyte detection have been incorporated here. The recent trends in advanced biosensors, such as smartphone interface for biosensing, nanozymes, lab-on-a-chip based detection methods have been discussed.

Biosensor nanoengineering: Design, operation, and implementation for biomolecular analysis

The use of field-Effect-Transistor (FET) type biosensing arrangements has been highlighted by researchers in the field of early biomarker detection and drug screening. Their non-metalized gate dielectrics that are exposed to an electrolyte solution cover the semiconductor material and actively transduce the biological changes on the surface. The efficiency of these novel devices in detecting different biomolecular analytes in a real-time, highly precise, specific, and label-free manner has been validated by numerous research studies. Considerable progress has been attained in designing FET devices, especially for biomedical diagnosis and cell-based assays in the past few decades. The exceptional electronic properties, compactness, and scalability of these novel tools are very desirable for designing rapid, label-free, and mass detection of biomolecules. With the incorporation of nanotechnology, the performance of biosensors based on FET boosts significantly, particularly, employment of nanomaterials such as graphene, metal nanoparticles, single and multi-walled carbon nanotubes, nanorods, and nanowires. Besides, their commercial availability, and high-quality production on a large-scale, turn them to be one of the most preferred sensing and screening platforms. This review presents the basic structural setup and working principle of different types of FET devices. We also focused on the latest progression regarding the use of FET biosensors for the recognition of viruses such as, recently emerged COVID-19, Influenza, Hepatitis B Virus, protein biomarkers, nucleic acids, bacteria, cells, and various ions. Additionally, an outline of the development of FET sensors for investigations related to drug development and the cellular investigation is also presented. Some technical strategies for enhancing the sensitivity and selectivity of detection in these devices are addressed as well. However, there are still certain challenges which are remained unaddressed concerning the performance and clinical use of transistor-based point-of-care (POC) instruments; accordingly, expectations about their future improvement for biosensing and cellular studies are argued at the end of this review.

Biosensing based on field-effect transistors (FET): Recent progress and challenges.

In the first wave of synthetic biology, genetic elements, combined into simple circuits, are used to control individual cellular functions. In the second wave of synthetic biology, the simple circuits, combined into complex circuits, form systems-level functions. However, efforts to construct complex circuits are often impeded by our limited knowledge of the optimal combination of individual circuits. For example, a fundamental question in most metabolic engineering projects is the optimal level of enzymes for maximizing the output. To address this point, combinatorial optimization approaches have been established, allowing automatic optimization without prior knowledge of the best combination of expression levels of individual genes. This review focuses on current combinatorial optimization methods and emerging technologies facilitating their applications.

Application of combinatorial optimization strategies in synthetic biology.

Transcription-factor-based biosensors (TFBs) are often used for metabolite detection, adaptive evolution, and metabolic flux control. However, designing TFBs with superior performance for applications in synthetic biology remains challenging. Specifically, natural TFBs often do not meet real-time detection requirements owing to their slow response times and inappropriate dynamic ranges, detection ranges, sensitivity, and selectivity. Furthermore, designing and optimizing complex dynamic regulation networks is time-consuming and labor-intensive. This Review highlights TFB-based applications and recent engineering strategies ranging from traditional trial-and-error approaches to novel computer-model-based rational design approaches. The limitations of the applications and these engineering strategies are additionally reviewed.

Transcription-Factor-based Biosensor Engineering for Applications in Synthetic Biology.

Biosensors are enabling major advances in the field of analytics that are both facilitating and being facilitated by advances in synthetic biology. The ability of biosensors to rapidly and specifically detect a wide range of molecules makes them highly relevant to a range of industrial, medical, ecological, and scientific applications. Approaches to biosensor design are as diverse as their applications, with major biosensor classes including nucleic acids, proteins, and transcription factors. Each of these biosensor types has advantages and limitations based on the intended application, and the parameters that are required for optimal performance. Specifically, the choice of biosensor design must consider factors such as the ligand specificity, sensitivity, dynamic range, functional range, mode of output, time of activation, ease of use, and ease of engineering. This review discusses the rationale for designing the major classes of biosensor in the context of their limitations and assesses their suitability to different areas of biotechnological application.
