I have developed "tennis elbow" from lugging this book around the past four weeks, but it is worth the pain, the effort, and the aspirin. It is also worth the (relatively speaking) bargain price. Including appendixes, this book contains 894 pages of text. The entire panorama of the neural sciences is surveyed and examined, and it is comprehensive in its scope, from genomes to social behaviors. The editors explicitly state that the book is designed as "an introductory text for students of biology, behavior, and medicine," but it is hard to imagine any audience, interested in any fragment of neuroscience at any level of sophistication, that would not enjoy this book. AIMS To investigate the relationships between three pain parameters (duration, intensity, and frequency), the number of pain sites and comorbidities, and the risk of having coexisting pain and/or comorbidities in patients with temporomandibular disorder (TMD) pain. METHODS The sample consisted of 198 outpatients attending the Dental Hospital of Chulalongkorn University. TMD pain was determined using the Diagnostic Criteria for TMD. Pain lasting 3 months or longer was defined as chronic pain. Pain intensity was reflected using a 0- to 10-point numeric rating scale, and pain frequency was assessed with the percentage of pain days over a 2-week period. The number of pain sites was evaluated using the Widespread Pain Index. The presence of comorbidities was assessed with a validated diagnostic questionnaire. The associations were analyzed using Spearman rho test, multiple linear regression, and logistic regression, with a significance level of P ≤ .05. Age and gender were analyzed as confounders. RESULTS The number of pain sites was related to pain duration, pain intensity, and age. The number of comorbidities was associated with pain duration. Neither pain frequency nor gender were related to the number of pain sites or comorbidities. When the pain duration reached 1 month, patients had a 1.045-times higher probability of pain beyond the orofacial area (odds ratio [OR] = 1.045; 95% confidence interval [CI] = 1.024 to 1.066; P = .001) and a 1.028-times higher probability of comorbidities (OR = 1.028; 95% CI = 1.005 to 1.05; P = .008). For an increase of 1 score on the numeric rating scale, patients had a 1.206-times higher probability of pain presence beyond the orofacial area (OR = 1.206; 95% CI = 1.068 to 1.344; P = .026). CONCLUSION High pain intensity and long pain duration increase the probability of having coexisting pain and comorbidities in TMD pain patients.

Pain Duration and Intensity Are Related to Coexisting Pain and Comorbidities Present in Temporomandibular Disorder Pain Patients.

Complex regional pain syndrome (CRPS) is characterized by ongoing pain, swelling, and stiffness following an acute injury. CRPS is difficult to diagnose, significantly impacts functioning, and is frequently incurable. Current treatments are pharmacotherapy, surgery, and physiotherapy. This case report describes the use of eye movement desensitization and reprocessing (EMDR) in the psychotherapeutic treatment of a woman diagnosed with CRPS in 2009 as a result of injuries sustained during an assault in 2004. This article reports on EMDR treatment provided 1–2 years after her diagnosis. At initial assessment, the client was debilitated and suicidal, unable to work or care for her children, and dependent on her family for financial support because of CRPS. Two phases of 7 EMDR sessions were provided; the first focused on past traumatic experiences; the second addressed her pain with Grant’s (2009) EMDR chronic pain protocol. At the end of treatment, the client reported decreased pain, decreased substance dependence, improved mood and outlook, and was able to resume part-time work. Results were maintained at 8-month follow-up and suggest that EMDR was helpful for this client in reducing the symptoms associated with CRPS.

EMDR as a Therapeutic Treatment for Complex Regional Pain Syndrome: A Case Report

Cisplatin is the most effective and neurotoxic platinum chemotherapeutic agent. It induces a peripheral neuropathy characterized by distal axonal degeneration that might progress to degeneration of cell bodies and apoptosis. Most symptoms occur nearby distal axonal branches and axonal degeneration might induce peripheral neuropathy regardless neuronal apoptosis. The toxic mechanism of cisplatin has been mainly associated with DNA damage, but cisplatin might also affect neurite outgrowth. Nevertheless, the neurotoxic mechanism of cisplatin remains unclear. We investigated the early effects of cisplatin on axonal plasticity by using non-cytotoxic concentrations of cisplatin and PC12 cells as a model of neurite outgrowth and differentiation. PC12 cells express NGF-receptors (trkA) and respond to NGF by forming neurites, branches and synaptic vesicles. For comparison, we used a neuronal model (SH-SY5Y cells) that does not express trkA nor responds to NGF. Cisplatin did not change NGF expression in PC12 cells and decreased neurite outgrowth in both models, suggesting a NGF/trkA independent mechanism. It also reduced axonal growth (GAP-43) and synaptic (synapsin I and synaptophysin) proteins in PC12 cells, without inducing mitochondrial damage or apoptosis. Therefore, cisplatin might affect axonal plasticity before DNA damage, NGF/trkA down-regulation, mitochondrial damage or neuronal apoptosis. This is the first study to show that neuroplasticity-related proteins might be early targets of the neurotoxic action of cisplatin and their role on cisplatin-induced peripheral neuropathy should be investigated in vivo.

Non-cytotoxic Concentration of Cisplatin Decreases Neuroplasticity-Related Proteins and Neurite Outgrowth Without Affecting the Expression of NGF in PC12 Cells.

Human perception has been likened to a hologram. A hologram exists by converging two or more laser beams together, producing a three-dimensional vision that is very real, but doesn’t really exist. You can put your hand right through a hologram, yet it is quite visible and not disturbed by your hand. The more laser beams we add to the hologram, the richer the vision. This analogy is often used to address human perception. What our brain creates as a perception and how we project these perceptions onto the outside world are called qualia. The qualia we call our conscious experience of pain cannot be fully explained by neurophysiological events only. Some qualia, or perceptions, can be up to 90 % memory. Thus, our qualia are produced by a dynamic interaction between mind and brain and most likely through the mechanics of quantum physics.

Pain as a Perceptual Experience

Neuroplasticity is a term that is used quite frequently these days in pain-related literature, and in many ways, it has come to be a term especially associated with maldynia. However, neuroplasticity is a term that more accurately delineates the way our nervous system operates, peripherally and centrally, and it should have no intrinsic judgment placed upon it. It simply is what it is.

Neuroplasticity, Sensitization, and Pain

Pain is a common complaint and its perception is a complex issue. The older person with neck and shoulder pain may have contributions to that pain from multiple and diverse sources. These can range from nociceptive stimulation, neurologic sensitization, emotional issues, socio-cultural biases, cognitive interpretation and meanings of the pain to that person, concurrent medical and psychiatric illnesses, and memory (both pain and non-pain related memories). The affective dimension of pain can be more influential on a person's ultimate pain experience than the sensory-discriminative component, and both must be understood for each patient, in terms of it's relative weight in each pain. Neck and shoulder pain can represent eudynia and maldynia, or concurrent existence of both. To properly treat patients with this complaint, physicians must understand what comprises each individual's pain hologram and direct treatment at as many component parts as possible.
