症例は54歳，女性．軽自動車同士の衝突事故で受傷．救急隊接触時は意識清明，歩行可能であったが，経時的に左前胸部が膨隆し，救急車内で血圧低下を認めた．来院時，左乳房に著明な腫脹と圧痛，皮下出血斑を認めた．血圧73/36mmHg，脈拍108回/分，呼吸数27回/分とショックバイタルであった．造影CTにて左乳房内に巨大血腫と造影剤の血管外漏出を認め，左外側胸動脈分枝からの活動性出血が疑われた．循環動態が不安定な状態での全身麻酔はリスクが高く，また血管内治療では出血点まで確実に到達できない可能性を考慮し，局所麻酔下で大胸筋前面の大量血腫を除去し，ガーゼと弾性包帯で圧迫固定を行った．術翌日に圧迫を解除し止血が得られていることを確認，術後6日目にドレーン抜去し，翌日退院となった．今回われわれは，局所麻酔下での血腫除去術および圧迫止血により治癒した外傷性乳房内出血症例を経験したので報告する．
