Utilization of long-lasting insecticidal nets (LLIN) has been associated with reduction of malaria incidence, especially among children. The 2013 Nigeria Demographic and Health Survey revealed Osun State had the least proportion (5.7%) of under-five children (U5) who slept under LLIN the night before the survey. A study was conducted to assess caregivers’ knowledge about LLIN, utilization of LLIN and factors influencing LLIN use among U5 in Osun State, Nigeria. A cross-sectional study was carried out among 1020 mothers/caregivers of U5 selected from six communities in Osun State using a multistage sampling technique. A pre-tested interviewer administered questionnaire was used to collect information on socio-demographic characteristics, mothers’ knowledge about LLIN, ownership and utilization of LLIN and factors influencing use of LLIN in U5. Questions on knowledge about LLIN were scored and categorized into good (scored ≥ 5) and poor (score < 5) knowledge out of a maximum obtainable score of seven. Utilization of LLIN was defined as the proportion of U5 who slept under net the night before the survey. Data were analysed using descriptive statistics, Chi square test and logistic regression at α < 0.05. Transcripts from focus group discussions (FGD) were analysed for emerging themes related to caregivers’ perspectives on utilization and factors affecting use of LLIN among U5. Majority of the respondents 588 (58.3%) fall between age 25–34 years, with a mean age of 30.0 ± 6.3 years. All were aware of LLIN but only 76.1% had good knowledge and 59.0% reported use of LLIN among their U5. Reported barriers to utilizing LLIN were; heat (96.4%), reactions to the chemical (75.5%) and unpleasant odour (41.3%). These were corroborated at FGD. Those with formal education [adjusted odds ratio (aOR) = 1.4; 95% CI 1.0–2.1] and those with good knowledge of LLIN (aOR = 1.8; 95% CI 1.4–2.5) were more likely to use LLIN than their counterparts without formal education and those with poor knowledge of LLIN respectively. The level of knowledge of respondents about LLIN was high and the utilization of LLIN among U5 was above average, however, it is still far below the 80% target. Efforts should be made to further improve utilization of LLIN through intensified promotion and health education.

Caregivers' knowledge and utilization of long-lasting insecticidal nets among under-five children in Osun State, Southwest, Nigeria.

Since 2008, Indoor Residual Spraying (IRS) has been performed in Benin in 19 districts, including 4 in southern Benin, 9 in Atacora, and 8 in Atacora, Alibori and Donga in northern Benin. However, Benin still struggles with questions about IRS cost–benefit and epidemiological impact. Lessons learned and challenges from 10 years of IRS in Benin to be shared with the stakeholders involved in vector control implementation for decision-making. Entomological parameters have been assessed entomological parameters in IRS communes since 2008. In all IRS intervention communes, decreases in human biting rate (HBR) of Anopheles gambiae, blood feeding inhibition and entomological inoculation rate (EIR) as compared to control district have been measured. EIR was reduced by 80–90%, which is encouraging, but should be observed with caution because: (i) the reduction may be insufficient to decrease epidemiological indicators given that the residual EIR in IRS districts is still higher than it is in some regions of stable malaria; (ii) the reduction in EIR is based on comparisons with control communes, but it is difficult to select control areas with the same environmental characteristics as intervention areas; (iii) despite the reduction, half of all mosquitoes that entered IRS-treated houses succeeded in taking human blood meals. Further, there are behaviours among Benin’s population that limit IRS efficacy, including recent data showing that > 90% of people are not protected by IRS between 7 and 10 p.m. This is due to the fact that they remain outdoors and that most people are not protected from mosquito bites after 10 p.m. because they either sleep outdoors without IRS protection or indoors without an ITN. Moreover, people have large amounts of clothing hanging on walls where mosquitoes can rest instead of IRS-treated walls. Finally, other components are important to consider in implementing IRS among which: (i) Vector resistance management strategies are sometimes poorly understood; this is actually different from the need to replace one insecticide with another after the emergence of resistance; (ii) African countries should prepare to finance IRS themselves. To curtail residual malaria transmission, additional interventions able to target vectors escaping IRS should be prioritized.

Lessons learned, challenges and outlooks for decision-making after a decade of experience monitoring the impact of indoor residual spraying in Benin, West Africa

Background: LLIN is one of the proven interventions adopted by the national malaria program to reduce malaria burden in Nigeria. Despite the marginal high ownership of LLIN across the geopolitical zones of Nigeria, utilization among the under-five children is low. The study aimed to identify determinants of household use of long-lasting insecticidal net among under-five children in Nigeria. Methods: A review of 2015 Malaria Indicator Survey was done. Independent variables include household’s and women socio-demographic and economic characteristics. Outcome variables of interest was LLIN use by persons in household. Univariate, bivariate and multivariate analysis done to identify predictors of HH LLIN use in under-five children. Level of significance was set at <0.05. Results: Twenty-five thousand six hundred and thirty-nine (66.7%) of under-five children slept under LLIN the night before the conduct of the survey. There was statistically significant association (p<0.05) between age, type of residence, wealth index, fever in the last two weeks before the conduct of the survey, number of mosquito bed nets and number of sleeping rooms. Conclusion: LLIN ownership among household members most especially the under-five children were still not optimal bearing in mind the need for universal coverage of LLIN. Predictors of LLIN use among under-five children include age, type of residence, wealth index, fever in the last two weeks before the conduct of the survey, number of mosquito bed nets and number of sleeping rooms.

Determinants of Household Long Lasting Insecticidal Nets Use Among Children Under 5 Years in Nigeria

Sleeping under insecticidal treated bed-nets has been proven as an effective tool towards preventing mosquito bites and malaria but the burden of malaria cases seems to be on the increase. This study therefore assessed the limiting factors associated with ownership and usage of Long-Lasting Insecticidal Nets (LLINs) in four periurban communities in Ogun State, Nigeria. A total of 499 residents above age 15, were recruited and interviewed at the healthcare facilities in the selected communities using a well-structured questionnaire. Data obtained were analyzed using SPSS 25.0 version for descriptive and chisquare statistics, with significance set at 95%. Of the 499 residents, 72.1% (360/499) owned at least one LLIN, while only 56.7% (204/360) affirmed to sleeping under the bed net. Heat was a major reason for non-usage, 70/135 (51.9%) followed by harboring dust 40/135 (29.6%), skin irritation 16/135 (11.9%) and problem of hanging nets 2/135 (1.5%). There were significance differences between the challenges in the use of LLIN by the respondents and the peri-urban communities (p= 0.000). Majority of the respondents also reported LLIN abuse; 59/93 (63.4%) used them as window or door nets, 13/93 (13.9%) and 13/93 (13.9%) used them as fishing nets and as sieves and sponges respectively. Other uses included using it as a means of protection against rodents on the farm and as football goal nets. There was no significance difference between the abuse of LLINs by the respondents and the peri-urban communities (p= 0.209). The non-usage and abuse of LLIN will dampen the efforts targeted at controlling malaria in the southwestern region of Nigeria. It is therefore important that malaria control programmes incorporate health awareness campaigns targeted at promoting usage and disapproving the abuse of LLINs

Coverage and usage of insecticidal treated bed-nets for preventing malaria in four peri-urban communities of Ijebu-North, Southwest Nigeria

BACKGROUND Malaria remains a major public health problem in Ethiopia. Consequently, Ethiopia designed the 2011-2015, Malaria Prevention and Control Strategic Plan to fight the vector. It was discovered that most of the studies conducted on the use of insecticide-treated nets (ITNs) were not in line with the strategic plan of the country. This study aimed to assess ITN ownership and utilization, and includes barriers related to its use among the target-area population at household (HH) level. MATERIALS AND METHODS A cross-sectional design was employed in Itang for this study. Data were collected by trained nurses through face-to-face interview and observation. A total of 845 participants were selected through multistage sampling, and the size was determined by using a single-population proportion formula. EPI Info and SPSS was used for analysis, and all necessary statistical association was computed in order to explain the outcome variable through explanatory variables of this study. RESULTS Among 845 HHs interviewed, 81.7% (690) had at least one ITN, while 52.3% (361) had used the ITN the night preceding the data-collection day. HH awareness of malaria prevention, number of ITNs, family size, number of family members sharing sleeping area/beds, sleeping patterns of adolescents, HH-head age, and inconvenience of using ITNs were found to be barriers to the use of ITNs in this study. CONCLUSION AND RECOMMENDATION The study concluded that very few HHs owned ITNs and there was very low usage of ITNs. In recommendation, the regional health bureau and district health office should consider bigger nets that can accommodate family members who share the same sleeping area/bed in the area.

Insecticide-treated net ownership and utilization and factors that influence their use in Itang, Gambella region, Ethiopia: cross-sectional study.

Long-lasting insecticidal nets (LLINs) have proved to be an important tool for the malaria control and other vector-borne diseases. Benin, by its National Malaria Control Program, conducted in July, 2011 a universal distribution campaign where approximately 5 million of nets were distributed. But after this mass-distribution, questions arise: Do people effectively use or not use the mosquito nets freely distributed? To clarify these questions, this study was conducted on LLINs coverage, use and physical condition in Benin one year after their distribution. The households were randomly selected from 32 clusters. Data on bed net ownership and usage, physical condition of the nets, other characters and issues related to sourcing were asked of all targets to assess the origins of LLIN found at the household level. Of the total surveyed, 88.96% had at least one LLIN. 87.19% of these nets come from the last campaign, 9.1% were from pregnant women voucher clinic delivery systems and 3.20% were purchased full price. 84% of the nets were reportedly used the night preceding the survey. Around 89% of the total of LLINs observed was in good condition. The universal distribution campaign in Benin has significantly increased LLIN coverage and ownership policy in the community.
