Resume Objectifs Relever l’ensemble des anomalies cardiaques decouvertes au cours des angioscanners thoraciques realises pour suspicion d’embolie pulmonaire (EP). Patients et methodes Etude retrospective portant sur 780 patients ayant ete explores par angioscanner pulmonaire avec determination des anomalies cardiaques notees associees ou non a une embolie pulmonaire. Resultats Des anomalies cardiaques ont ete notes dans 80 cas, associees dans uniquement 20 % des cas a une embolie pulmonaire. Les anomalies les plus frequentes etaient la dilatation des cavites cardiaques et les calcifications coronaires. Conclusion Il est important de verifier sur un angioscanner pulmonaire s’il existe ou non des anomalies cardiaques pouvant expliquer une symptomatologie thoracique en l’absence d’embolie pulmonaire.

Examen non invasif : L'angioscanner pulmonaire est un examen non invasif dont les seules contre-indications relatives sont l'insuffisance renale et l'allergie a l'iode. Embolie pulmonaire massive : En cas de suspicion d'une telle embolie, l'angioscanner est l'examen ideal en raison de son excellente performance pour le diagnostic d'embolie pulmonaire proximale et de son innocuite. Embolie pulmonaire non massive : Dans la strategie diagnostique, l'angioscanner pourrait, en raison de sa specificite elevee, etre l'examen de premiere intention a la place de la scintigraphie. Si l'angioscanner montre un thrombus, le diagnostic d'embolie pulmonaire est confirme et un traitement institue. Si l'angioscanner ne montre pas de thrombus, le diagnostic d'embolie pulmonaire peut etre ecarte dans la majorite des cas. En cas de doute, d'autres investigations non invasives doivent etre pratiquees. Le recours a l'angiographie devrait devenir exceptionnel.
