Emollients are an integral part of the therapy for patients with atopic dermatitis. The most effective will be the new generation of emollients – emollients plus, which normalize the skin microbiome. Their effectiveness can be assessed by determining the parameters of the epidermal barrier (hydratation, sebum level, pH) and skin elasticity. Objectives. To evaluate the effectiveness and safety of a lipid-restoring cream containing a biotic complex LE SANTI in children with atopic dermatitis. Material and methods. From August to December, 25 children (9 girls, 36 % and 16 boys, 64 %) aged from 3 to 17 years (average age 9.2 years) with atopic dermatitis participated in an observational program to evaluate the effectiveness and safety of a lipid-restoring cream LE SANTI. For 21 days, patients applied the cream twice a day, they had 6 visits, during which was collected their medical history, adverse events, physical examination, determined indicators of the epidermal barrier (hydratation, sebum content, pH), biomechanical characteristics (skin elasticity), and also severity of disease (SCORAD) and quality of life (CDLQI). At visit 6, patients were assessed for satisfaction with the use of the cream. Results. All participants of the observation program completed their participation on time. As part of the observational program, there was an increase in skin hydration and sebum level by 3 and 3.5 times, respectively, pH level turned to the acidic values by 18%, an increase in elasticity by 30%, a decrease in SCORAD and CDLQI indices by 1.9 times or more 3 times respectively. The patients did not experience any adverse events; the satisfaction rating from the use of the cream was 4.78/5. Conclusions. Lipid-restoring cream LE SANTI can be recommended for inclusion in the treatment regimen for atopic dermatitis as a basic external agent that meets all the requirements for effectiveness and safety for children with atopic dermatitis, as well as basic care during remission. 

Change of epidermal barrier parameters and biomechanical parameters of the skin in children with atopic dermatitis during the use of a lipid-restoring cream containing biotic complex

The article presents the results of an expert council to discuss the relationship between disorders of the skin microbiome and the course of atopic dermatitis, as well as the key mechanisms of action on the skin of a complex of pro- and prebiotics in dermatocosmetics for use in patients with dermatoses from the perspective of modern clinical protocols. 

Skin microbiome and atopic dermatitis (Resolution of the Council of Experts)

Введение. Кожа пациентов с атопическим дерматитом имеет отличный от здоровых индивидуумов микробиом, а также характеризуется более высокой обсемененностью Staphylococcus aureus. Восстановление нормального соотношения микроорганизмов на поверхности кожи улучшает резистентность пациентов с атопическим дерматитом к инфицированию, а также способствует нормализации эпидермального барьера. Это объясняет целесообразность применения в терапии атопического дерматита топических средств, стабилизирующих микробиом кожи пациентов. Цель. Оценить эффективность и безопасность применения эмолиента – крема липидовосстанавливающего, содержащего биотический комплекс (BIOTIC COMPLEX: lactobacillus plantarum heal 19, олигосахарид и инулин), у детей с легким и среднетяжелым течением атопического дерматита. Материалы и методы. С декабря 2022 по март 2023 г. на базе клиники кожных болезней Санкт-Петербургского государственного педиатрического медицинского университета в наблюдательную программу были включены 28 детей в возрасте от 5 до 17 лет с легким и среднетяжелым течением атопического дерматита. Все пациенты получали терапию согласно федеральным клиническим рекомендациям по лечению атопического дерматита, а в качестве топической терапии применяли крем липидовосстанавливающий, содержащий биотический комплекс (BIOTIC COMPLEX), увлажняющие и смягчающие компоненты, а также разрешенный к применению у детей с 0 лет. Наблюдательная программа включала 4 визита (1-й, 3-й, 7-й и 14-й дни). На 1-м и 4-м визитах у пациентов была проведена оценка степени тяжести атопического дерматита с помощью индекса SCORAD, на каждом визите у пациентов проводили измерение показателей эпидермального барьера (увлажненность, pH, себуметрия) с помощью аппарата Multi Skin Test Center 750 (Германия). Результаты. На первом визите среднее значение SCORAD у пациентов составило 26,4, в то время как на завершающем визите – 9,3. У всех участников программы (n=28; 100%) в 1-й день наблюдалось умеренное шелушение кожи и зуд, у 8 (29%) – отмечались очаги лихенификации и эритемы. Уровень рН кожи на первом визите составил 6,73±0,46, увлажненность кожи – 30,8±5,4, содержание кожного сала – 0,6±0,2. На 7-й день (визит 3) у 26 пациентов (93%) не было шелушения, среднее значение рH кожи составило 6,11±0,27; гидратация кожи – 42,6±5,2; содержание кожного сала – 1,2±0,3. На последнем визите у всех пациентов не было обнаружено объективных признаков сухости кожи. Среднее значение SCORAD составило 9,3; рН кожи – 5,5±0,72; гидратация кожи – 51,6±4,7; содержание кожного сала – 1,6±0,6. 26 из 28 пациентов (93%) отметили полное отсутствие зуда за период применения крема липидовосстанавливающего с содержанием биотического комплекса (BIOTIC COMPLEX). В рамках проведения наблюдательной программы не было зарегистрировано побочных явлений, связанных с применением эмолиента, у пациентов. Заключение. По результатам динамической оценки отмечалось снижение индекса степени тяжести атопического дерматита в 2,8 раза у пациентов, применявших липидовосстанавливающий крем дважды в день. Разрешение объективных и субъективных проявлений атопического дерматита у подавляющего большинства пациентов отмечалось уже к 7-му дню применения крема. По завершении наблюдательной программы у пациентов было зарегистрировано полное восстановление показателей увлажненности и кислотности кожи.
 Introduction. The skin of patients with atopic dermatitis has a different microbiome than healthy individuals, and is also characterized by a higher contamination of Staphylococcus aureus. Restoration of the normal ratio of microorganisms on the skin surface improves the resistance of patients with atopic dermatitis to infection, and also contributes to the normalization of the epidermal barrier. This explains the expediency of using topical agents in the treatment of atopic dermatitis that stabilize the skin microbiome of patients. Objectives. To evaluate the efficacy and safety of using an emollient containing a biotic complex (BIOTIC COMPLEX: lactobacillus plantarum heal 19, oligosaccharide and inulin) in children with mild and moderate atopic dermatitis. Materials and methods. From December 2022 to March 2023, 28 children aged 5 to 17 years with mild and moderate atopic dermatitis were included in the observational program on the basis of the clinic of skin diseases of the St. Petersburg State Pediatric Medical University. All patients received therapy in accordance with federal clinical guidelines for the treatment of atopic dermatitis, and as a topical therapy they received a lipid-restoring cream containing a biotic complex, moisturizing and emollient components, and also approved for use in children from 0 years of age. The observation program consisted of 4 visits (1, 3, 7 and 14 days). At the first and fourth visits, the patients were assessed for the severity of atopic dermatitis using the SCORAD index, and at each visit, the epidermal barrier parameters (hydratation, pH, sebumetry) were measured in patients with "Multi Skin Test Center 750" (Germany). Results. Patients had a mean SCORAD of 26.4 at the first visit, while 9.3 at the final visit. All participants in the study (n=28; 100%) had moderate desquamation and itching on day 1, 8 (29%) had areas of lichenification and erythema. The pH level of the skin at the first visit was 6.73±0.46, skin hydratation – 30.8±5.4, sebum content – 0.6±0.2. On the 7th day of the study, 26 patients (93%) had no desquamation, the average skin pH was 6.11±0.27; skin hydration – 42.6±5.2; sebum content – 1.2±0.3. At the last visit, no objective signs of dry skin were found in all patients. The mean SCORAD was 9.3; skin pH – 5.5±0.72; skin hydration – 51.6±4.7; sebum content – 1.6±0.6. 26 out of 28 patients (93%) noted the complete absence of itching during the period of application of the lipid-restoring cream containing the biotic complex. During the study, there were no side effects associated with the use of the emollient in patients. Conclusion. According to the results of the dynamic assessment, there was a decrease in the severity index of atopic dermatitis by 2.8 times in patients who applied the lipid-restoring cream twice a day. The resolution of objective and subjective manifestations of atopic dermatitis in the vast majority of patients was noted already by the 7th day of applying the cream. At the end of the observational program, the patients showed a complete recovery of skin hydration and acidity.

Experience of Usage of the Lipid Restoring Cream LE SANTI Containing Biotic Complex (BIOTIC COMPLEX) in Children with Mild and Moderate Atopic Dermatitis

На сегодняшний день в мире отмечается пропорциональный рост числа людей, страдающих ожирением и псориазом. Оба заболевания признаны ассоциированными патологиями, причем сопровождающимися двунаправленным отрицательным влиянием, которое проявляется в виде взаимного усугубления и увеличивает вероятность манифестации других коморбидных состояний. Генетический базис двух нозологий различен, при этом определено четкое представление о генетической структуре псориаза, в то время как у пациентов с ожирением генетическая основа и ее влияние на формирование фенотипа изучены не в полной мере. Иммунологические и эндокринологические патогенетические аспекты псориаза и ожирения имеют связующее звено в виде перманентного воспалительного фона, повышенной продукции TNF-α, IL-1, -12, -17, -23, лептина. Высокий уровень лептина отмечается у пациентов с ожирением, у пациентов с тяжелым течением псориаза и с нормальной массой тела, а также у пациентов с наличием двух заболеваний при различной степени тяжести псориаза. Лептин – важное связующее звено между эндокринологическими и иммунологическими процессами при псориазе и ожирении: белок вырабатывается адипоцитами, регулирует эндокринологические процессы и запускает системную вялотекущую воспалительную реакцию в организме. Так, пациенты с псориазом и ожирением формируют отдельный класс пациентов, которые нуждаются в особом мультидисциплинарном подходе. На сегодняшний день нет документов, официально регулирующих терапевтический алгоритм для данной категории пациентов. Врачи могут опираться на опыт коллег и данные, полученные из литературы, потому для практикующих специалистов доступность систематизированных данных очень важна. Классические методы борьбы с псориазом – топическая терапия и базовая системная терапия – имеют ряд ограничений. Даже самые современные генно-инженерные биологические препараты бывают недостаточно эффективными у пациентов данной категории, поэтому нормализация веса и снижение жировой массы – чрезвычайно важная цель в рамках курации пациентов с псориазом и ожирением.
 Today, there is a proportional increase in the number of people suffering from obesity and psoriasis in the world. Both diseases are recognized as associated pathologies, accompanied by a bidirectional negative effect, which manifests in the form of mutual aggravation and increases the possibility of other comorbid conditions manifestation. The genetic basis of nosologies is different. The genetic structure of psoriasis is clearly understood, while in patients with obesity, the genetic basis and its influence on the phenotype formation is unclear. Immunological and endocrinological pathogenetic aspects of psoriasis and obesity linked through the permanent inflammatory background, increased production of TNF-α, IL-1, -12, -17, -23, leptin. High levels of leptin are observed in patients with obesity, in patients with severe non-obese psoriasis, as well as in patients with the presence of two diseases with varying degrees of psoriasis severity. Leptin is an important link between endocrinological and immunological processes in psoriasis and obesity: the protein is produced by adipocytes, regulates endocrinological processes and triggers a systemic sluggish inflammatory response in the body. Thus, people with psoriasis and obesity form a special class of patients who need a multidisciplinary approach. To date, there are no papers officially regulating the therapeutic algorithm for this category of patients. Physicians can base on the experience of colleagues and data obtained from the literature, therefore, for practitioners, the availability of systematic data is very important. Classical options for psoriasis treatment – topical therapy and basic systemic therapy – have limitations. Even the most modern genetically engineered biological preparations are not effective enough in this category of patients, so the normalization of weight and reduction of fat mass is an extremely important goal in the curation of patients with psoriasis and obesity.

Линеарный огибающий ихтиоз Комеля – крайне редкое изолированное заболевание кожи, проявляющееся серпигинозными и мигрирующими эритематозными бляшками с ободком шелушения по периферии. Триада, включающая ихтиоз (у большинства – линеарный огибающий ихтиоз), изменения волос по типу Trichrrhexis invaginata и атопию, характеризует синдром Нетертона. На сегодняшний день нет сообщений об успешном лечении данного заболевания. Только благодаря современным возможностям медицинской генетики и лабораторной диагностики удалось установить патогенетические особенности заболевания, которые обусловлены мутацией гена SPINK5. Наличие мутации приводит к формированию дисфункции кожного барьера у пациентов, в результате чего при проникновении экзогенных антигенов в кожу формируется неспецифическое воспаление и активируется воспалительный ответ по Th2-пути. В мировой научной литературе опубликованы немногочисленные данные о лечении пациентов с заболеваниями из группы врожденных ихтиозов с применением дупилумаба. Полученные данные позволяют провести аналогию с патофизиологическими процессами при атопическом дерматите, в лечении которого наиболее эффективным сегодня является ингибитор IL-4, 13. Авторы наблюдали хороший клинический ответ у пациентов с заболеваниями из группы врожденных ихтиозов. Приводим описание успешного лечения пациента с линеарным огибающим ихтиозом Комеля в составе синдрома Нетертона с помощью ингибитора IL-4, 13 в педиатрической дерматологической практике.
 Linear circumflex ichthyosis of Comel is an extremely rare isolated skin disease that manifests as serpiginous and migrating erythematous patches with double-edged scales. The triad of ichthyosis (often – linear circumflex ichthyosis), Trichorrhexis invaginata and atopy characterize Netherton syndrome. There are no reports of successful treatment of this disease. Due to the possibilities of medical genetics and laboratory diagnostics, it was possible to establish the pathogenetic features of the disease, which are caused by the mutation of the SPINK5 gene. This mutation leads to skin barrier dysfunction in patients, as a result exogenous antigens penetrate the skin, form there nonspecific inflammation and the activates inflammatory response mediated by Th2 pathway. Few data have been published in the world scientific literature about the treatment of patients with diseases from the group of congenital ichthyosis using dupilumab. Obtained data allow us to draw an analogy with the pathophysiological processes in atopic dermatitis, in the treatment of which the IL-4, 13 inhibitor is the most effective today. The authors observed a good clinical response in patients with diseases from the group of congenital ichthyosis. We describe the successful treatment of a patient with linear circumflex ichthyosis as a part of Netherton syndrome with an IL-4, 13 inhibitor in pediatric dermatological practice.
