Chiari Malformation Type 1: A Systematic Review of Natural History and Conservative Management

Complications of posterior cranial fossa decompression for Chiari malformations are uncommon. We present three patients who developed infratentorial supracerebellar hygromas causing acute hydrocephalus after posterior cranial fossa decompression.

Acute Hydrocephalus following a Chiari I Decompression

Object. Chiari-syringomyelia is a heterogeneous condition that may be treated by decompression of the foramen magnum. Raised intracranial pressure (ICP) and/or hydrocephalus is a rare complication of this treatment. We aim to describe the incidence, clinical presentation, radiographic findings, management and outcome of patients developing raised ICP and/or hydrocephalus after hindbrain decompression for Chiari I malformation. Methods. Retrospective analysis of 138 consecutive adult and paediatric patients with Chiari I malformation who underwent foramen magnum decompression. Results. The incidence of post-operative symptomatic raised ICP and/or hydrocephalus in this series was 8.7%. Overall, 9 of 12 patients developing raised ICP or hydrocephalus required a VP shunt, an overall incidence of 6.5%. Raised intracranial pressure and hydrocephalus following hindbrain decompression for Chiari I malformation: a case series and review of the literature.

Obstructive hydrocephalus complicating foramen magnum decompression (FMD) for a Chiari malformation (CM) Type I is rare. Two female patients (17 and 55 years old) presented with strain-related headaches. In both cases magnetic resonance (MR) imaging studies confirmed a CM Type I, which was accompanied by syringomyelia in one case. Both patients underwent uncomplicated FMD with good initial recovery. Unfortunately, conditions in both patients deteriorated, with severe headaches occurring between Days 5 and 6 post-FMD. Decreased consciousness occurred in one case. In both patients, computerized tomography scanning demonstrated an acute obstructive hydrocephalus associated with bilateral infratentorial extraaxial fluid collections (EAFCs). In addition, left parafalcine and convexity EAFCs were present in one case. An emergency external ventricular drain was required in one patient, with delayed conversion to a ventriculo-peritoneal shunt. Spontaneous resolution occurred in the other patient without neurosurgical intervention. In both cases, MR imaging confirmed that each EAFC was subdural, resembled cerebrospinal fluid (CSF), and had distorted the superior cerebellum anteroinferiorly. Despite upper fourth ventricle/aqueduct compromise in one case, normal aqueduct flow artifacts were apparent on examination. All EAFCs resolved spontaneously. Obstructive hydrocephalus complicating FMD is rare but invariably associated with infratentorial EAFCs, which were confirmed to be subdural hygromas in this report. The authors assert that hydrocephalus results from upper fourth ventricle/aqueduct compromise as a result of CSF subdural dissection following a pinhole arachnoid tear on durotomy. Because such hygromas spontaneously resolve, permanent shunt insertion should be avoided.

Acute obstructive hydrocephalus associated with infratentorial subdural hygromas complicating Chiari malformation Type I decompression. Report of two cases and literature review.

The occurrence of subdural cerebrospinal fluid collections and ventricular dilatation (external hydrocephalus) after foramen magnum decompression is extremely rare. The authors report on a 37-year-old man who developed symptomatic subdural fluid collections (SFC) after uncomplicated foramen magnum decompression for Chiari I–syringomyelia complex. Postoperative magnetic resonance imaging revealed a supratentorial SFC with prominent midline shift. In addition, bilateral infratentorial SFCs extending supratentorially through the tentorial notch, pseudomeningocele and initial shrinkage of the syrinx were observed. Few days after evacuation of supratentorial collection, the patient experienced increasing headache and a computerized tomography scan demonstrated a contralateral subdural collection as well as ventricular dilatation. A programmable ventriculoperitoneal shunt was placed with resolution of supratentorial collection and progressive improvement of infratentorial collections and pseudomeningocele. Although previous reports described the occurrence of extra-axial fluid collections and hydrocephalus after foramen magnum decompression for Chiari malformation, to our knowledge, this is the only report of acute external hydrocephalus after foramen magnum decompression requiring urgent evacuation of SFC in order to reduce the mass effect.

Acute external hydrocephalus complicating craniocervical decompression for syringomyelia-Chiari I complex: case report and review of the literature.

Pseudomeningocele is a well known complication of foramen magnum decompression. Symptomatic subdural hygroma has not previously been described. We discuss a case of Arnold Chiari malformation type 1 in a 55-year-old Caucasian woman who underwent foramen magnum and upper cervical decompression and who developed a symptomatic subdural hygroma 2 weeks following surgery. This complication was most likely due to a technical error during surgery. It responded favourably to conservative therapy.
