Chronic inflammation and cumulative oxidative stress have been theorized as two common pathways of the interconnection between periodontitis and diabetes. Improvement in oxidizing status has been demonstrated in periodontal patients with diabetes treated with proper non-surgical periodontal treatment. In addition to periodontal treatment, Gaseous ozone therapy has been reported to possess anti-inflammatory properties and the ability to stimulate the endogenous antioxidant defence mechanism. To date, the antioxidant effect of gaseous ozone, in addition with periodontal treatment in diabetic patients, has been examined in only one study. The aim of this study was to determine the efficacy of gaseous ozone therapy as an alternative approach to supporting non-surgical periodontal therapy (NSPT), aimed at improving antioxidant machinery and interfering with ROS production on plasma levels in diabetic individuals diagnosed with moderate or severe periodontitis.One hundred and eighty patients with periodontitis and type 2 diabetes mellitus were randomly assigned to receive non-surgical periodontal treatment (NSPT) plus gaseous ozone therapy (A) NSPT alone (B). Clinical and periodontal parameters -Bleeding on probing (BOP), Periodontal pocket depth (PPD), and Clinical attachment Level (CAL)- and plasma levels of oxidant-antioxidant (TOS- TAOS) levels, glutathione (GSH), and malondialdehyde (MDA) were recorded at baseline and at 3- (T1) and at 6-months (T2) after treatment.Both treatments were efficacious in reducing clinical parameters. However, there were no significant differences regarding oxidative stress parameters in group A compared to group B.In the present study, gaseous ozone therapy did not enhance the effect of periodontal treatment in reducing oxidative stress in plasma levels of periodontitis patients with type II diabetes.The study was registered with ISRCTN1728169 (23/07/2022).

Research efficacy of gaseous ozone therapy as an adjuvant to periodontal treatment on oxidative stress mediators in patients with type 2 diabetes: a randomized clinical trial

This study evaluated the effect of ultrasonic and manual scaling using different scaler materials on the surface topography of computer-aided designing and computer-aided manufacturing (CAD/CAM) ceramic compositions. After scaling with manual and ultrasonic scalers, the surface properties of four classes of CAD/CAM ceramic discs: lithium disilicate (IPE), leucite-reinforced (IPS), advanced lithium disilicate (CT), and zirconia-reinforced lithium silicate (CD) of 1.5 mm thickness were evaluated. Surface roughness was measured before and after treatment, and scanning electron microscopy was used to evaluate the surface topography following the performed scaling procedures. Two-way ANOVA was conducted to assess the association of the ceramic material and scaling method with the surface roughness. There was a significant difference in the surface roughness between the ceramic materials subjected to different scaling methods (p < 0.001). Post-hoc analyses revealed significant differences between all groups except for IPE and IPS where no significant differences were detected between them. CD showed the highest surface roughness values, while CT showed the lowest surface roughness values for the control specimens and after exposure to different scaling methods. Moreover, the specimens subjected to ultrasonic scaling displayed the highest roughness values, while the least surface roughness was noted with the plastic scaling method.

We ink this last editorial as a long, tiring yet fruitful journey comes to an end. Our hearts are filled with gratitude as we pen down what these four years truly meant. The journey in last four years had been a bumpy ride with COVID-19 taking a toll on each one of us. These were the testing times where we learnt more from real experiences than textbooks. The world had come to a halt but JISP hadn’t. It was taxing to get a task done with everything being shut and everyone working from home and shifting their working style to the digital world. COVID taught us that “Teams that stick together emerge as survivors of a sinking ship”. We (JISP) emerged as survivors, notwithstanding the situation. Those days’ submissions reached a soaring 900 and beyond. Neither the response to the authors was late, because of huge submissions nor a single issue was delayed, backlogged or compromised. Articles were pouring in especially related to COVID-19. It was as if everyone everywhere was a someone who knew all about the pandemic. With institutions being shut and inactive, we were floating in a bubble of guidelines on how to manage oral conditions associated with COVID-19. Though as much as it was tempting to publish such unproven, unsubstantiated, non-scientific data, JISP refrained or in fact resisted such publications as that would mean jeopardizing the standard of the journal even though many acclaimed societies were encouraging them. For the first time ever, JISP was fortunate enough to have published three Good Clinical Practice (GCP) Guidelines - 1. Management of periodontal disease in patients with diabetes,[1] 2- Recommendations for the management of dentin hypersensitivity[2] and 3- Recommendations for gum care.[3] These guidelines will act as pillars of learning and a reference for all. The guidelines for management of periodontal disease in patients with diabetes[1] were a joint statement of Indian Society of Periodontology and Research Society for the Study of Diabetes in India. Additionally, these guidelines were also published simultaneously in the Journal of the Research Society for the Study of Diabetes in India (RSSDI), the largest society of diabetologists in Asia.[4] JISP’s long 10 year plus association with the publishing house Medknow deserves applause. Their constant support and upgraded strategies has aided JISP to reach the level that it is at today. Our journal, being one of the top journals under their umbrella of 480 journals, was among the first few to be shifted to latest platform of Lippincott Williams and Wilkins with a new and more interactive website. The novel interactive website with alt-metrics provides detailed information about each article, it’s authors, citations, access throughout the world rendering the publication process efficient and simple for all. A new updated submission platform with electronic copyright form facility was also launched. The journal ‘Journal of Indian Society of Periodontology’ was indexed in SCOPUS since 2011. As a part of the SCOPUS re-evaluation initiative, SCOPUS selected JISP for revaluation and our SCOPUS indexation was renewed in 2020. Additionally JISP has also been included on the OVID platform. We receive submissions from more than 85 countries, making it truly an international journal in the real sense and deemed as one of the most prestigious journals. The readers, authors, reviewers of JISP deserve a special mention and we are truly indebted to them for the obvious reason that JISP exists “for and because” of them. It has been an incredible life changing journey as we bid adieu to an extraordinary team of amazing and talented people. Dr. Grover, our Hon. Secretary, has provided us continuous support, guidance, and has meticulously also managed the finances of JISP. We are truly indebted to him. Dr. Ashish Jain, whose out of the box thinking and excellent ideas (GCPs being his ideas) has helped the journal to reach new heights. Please accept our sincerest appreciation for your guidance both on a professional and personal front. Oral health is vital for the general well-being, overall body health and the quality of life throughout our lifetime. Most of the oral diseases and conditions are largely related to the maintenance of oral hygiene, lacking that people suffer from diverse oral diseases at different times in their life. With greater life expectancy, the individuals have teeth involved with periodontal diseases that would not only require professional care but also home gum care for them to last a lifetime. Indian Society of Periodontology (ISP) has recognized the need for systematic documents to update everyday clinical practice of general dental practitioners and have provided evidence-based consensus documents, namely good clinical practice recommendations from time to time to raise the oral health-related awareness and standards of oral healthcare delivery across the country. The current set of clinical practice recommendations focused to “Gum Care for All”, is aimed at emphasizing and enhancing the awareness regarding oral health promotion, maintenance, and disease prevention. Twenty-five subject matter experts from across the nation, prepared these recommendations after a thorough literature review and group discussions. The document has been prepared in three distinct sections, namely pretherapeutic, therapeutic, and post therapeutic, to provide guidance during the respective phases of patient management and may serve as a quick and concise reference to the readers. The guidelines shall provide the distinct definitions, signs and symptoms, treatment required; recall visit specifications for plausible clinical case situations, home care advice regarding maintenance of oral hygiene including information on brushing technique, care and change of brush, use of interdental aids, and mouthwashes, etc. The document should advocate and guide the combined efforts of general dentists, and the population at large toward an empowered, evidence based, integrated, and comprehensive oral health care, which shall enhance the healthful functioning and longevity of the dentition and general health of the individual.

ISP good clinical practice recommendations for gum care

Background: Periodontal disease is an inflammation caused by host parasite interaction leading to the destruction of tooth supporting structures. During inflammation there is an elevated levels of biochemical signalling molecules. A disintegrin and metalloproteinase 8 (ADAM8) molecule has been implicated in various physiologic functions and pathologies. The expression of ADAM8 is upregulated in inflammatory conditions, and with upregulated expression they play a pivotal role in inflammation, immunity and osteoclastogenesis. Objective: To evaluate ADAM8 levels in gingival crevicular fluid (GCF) collected from healthy individuals and individuals with periodontal disease at baseline and in the periodontal disease group after nonsurgical periodontal therapy. Methods: In total, 48 subjects aged 20–65 years were divided into separate groups: a periodontally healthy group (group 1: males, n = 7, females, n = 9), a chronic gingivitis group (group 2: males, n = 6, females, n = 10), and a chronic periodontitis group (group 3: males, n = 8, females, n = 8). GCF samples were collected from all the groups at baseline and 3 months after nonsurgical periodontal therapy in the chronic gingivitis (group 4) and chronic periodontitis (group 5) cases. Statistical analysis was performed using Shapiro-wilk normality test, One way ANOVA test, Kruskal-Wallis test followed by Mann-Whitney U test, Spearman’s rank correlation test and multiple linear regression analysis was done. The level of significance was determined at P < 0.05. Results: ADAM8 levels in the gingival crevicular fluid was significantly higher in group 3 (26,416.25 ± 7,817.59) than groups 1 and 2 at baseline at P < 0.001. After non-surgical periodontal therapy, ADAM8 levels in the gingival crevicular fluid was significantly reduced for group 2 (13,186.88 ± 3,247.62) and group 3 (18,375.63 ± 3,339.07) at P < 0.001. Conclusion: ADAM8 levels were increased in chronic gingivitis (group 2) and chronic periodontitis group (group 3) and reduced after non-surgical periodontal therapy in groups 2 and 3.
