The management of prolonged seizures (PS) and seizure clusters (SC) is impeded by the lack of international, evidence-based guidance. We aimed to develop expert recommendations regarding consensus definitions of PS, SC, and treatment goals to prevent progression to higher-level emergencies such as status epilepticus (SE). 

Outpatient management of prolonged seizures and seizure clusters to prevent progression to a higher‐level emergency: Consensus recommendations of an expert working group

Aim The aim of this study is to investigate the safety practices used by MRI departments in Saudi Arabia. Methods A cross-sectional study across 113 private and public hospitals was conducted in Saudi Arabia. A survey questionnaire was designed and sent to 113 MRI units. The questionnaire consisted of 43 items under 14 sections for the assessment of MRI safety practices. These 14 sections are related to (i) MRI and its safety, (ii) MRI usage and its safety, and (iii) safety of the MRI technologists and reporting of adverse effects during the usage. The American College of Radiology (ACR) guidance document on MRI safety practices was used as a template for this survey. Data were analyzed using IBM SPSS Statistical software for Windows version 26.0 (IBM Corp., Armonk, NY, USA). Results Of the 43 items assessed, only 3 items’ binary responses (Yes & No) did not differ much. A greater proportion of positive responses for 40 items (93%) regarding MRI safety practices. More than 50% of the participants claimed that their departments lacked a Magnetic Resonance Safety Officer (MRSO). Regarding regular safety training programs, less than 50% received training in MRI safety. Handheld metal detectors were found in only 39% of the MRI units. Conclusion The majority of MRI units in Saudi Arabia have demonstrated compliance with majority of ACR MRI safety recommendations; nonetheless, there are two main items for which the guidelines may not be attained: MRSO and regular MRI safety training programs. By taking into account the limitations of this study, it is strongly recommended to assign MRSO and implement annual MRI safety training to improve MRI safety practices for both patients and healthcare workers.

Assessment of MRI Safety Practices in Saudi Arabia

OBJECTIVE
Status epilepticus (SE) is frequently associated with peri-ictal magnetic resonance imaging (MRI) abnormalities (PMA). However, the anatomical distribution of these alterations has not been systematically studied. The aim of this study was to assess the localization patterns of PMA in patients with SE.


METHODS
In this prospective case-control study, we compared the distribution and combinations of diffusion-restricted PMA to diffusion-restricted lesions caused by other neurological conditions. All patients of the SE group and the control group underwent MRI including a diffusion-weighted imaging sequence. Patients with SE were imaged within 48 h after its onset.


RESULTS
We enrolled 201 patients (51 with SE and 150 controls). The most frequent locations of PMA in SE were cortex (25/51, 49%), followed by hippocampus (20/51, 39%) and pulvinar of thalamus (10/51, 20%). In the control group, the cortex was involved in 80 of 150 (53%), white matter in 53 of 150 (35%), and basal ganglia in 33 of 150 (22%). In the control group, the pulvinar of thalamus was never affected and hippocampal structures were rarely involved (7/150, 5%). Involvement of the pulvinar of thalamus and the hippocampus had high specificity for SE at 100% (95% confidence interval [CI] = 98-100) and 95% (95% CI = 91-98), respectively. The sensitivity, however, was low for both locations (pulvinar of thalamus: 20%, 95% CI = 10-33; hippocampus: 39%, 95% CI = 26-54).


SIGNIFICANCE
Diffusion-restricted MRI lesions observed in the pulvinar of thalamus and hippocampus are strongly associated with SE. These changes may help physicians in diagnosing SE-related changes on MRI in an acute setting, especially in cases of equivocal clinical and electroencephalographic manifestations of SE.

Magnetic resonance imaging fingerprints of status epilepticus: A case-control study.

Objective A likely, but less investigated interaction of the switched gradient field with medical implants is gradient-induced vibration [1], [2]. Vibrations are caused by induced local magnetic fields due to eddy-currents in electrically conductive structures of medical devices counteracting with the static main magnetic field. Measurement of vibrations in an MR environment is a complex experiment, especially if not contacting objects mechanically and thus preventing influence to the measurement. Therefore it is a desire using a contact-free measurement system. We describe an investigation using an optical sensor for measuring gradient-induced vibration on copper and titanium circular plates (dummy implants of a pacemaker can) as well as a real hip stem implant.

OBJECTIVE
MRI (Magnetic Resonance Imaging) does not use ionizing radiation and provides a comparatively better resolution. It is an important tool for the diagnosis of problems related to the head and neck area. It has various applications in dentistry, including MRI-based planning for implant placement. Previously, studies have been performed to assess its medical use, but very few studies have been conducted on its applications in dentistry.


MATERIALS AND METHODS
A pre-validated online questionnaire was distributed through various messenger groups and social media. The questionnaire comprised two sections to collect demographics and assess the knowledge and awareness among dentists about the interactions between dental prostheses and materials with MRI.


RESULTS
In the present study, 63.20% of respondents indicated that MRI provided a good view of the implant sites, TMJ and salivary gland tumours, but the majority believed that metallic and titanium implants do not interact or cause artefacts on MRI imaging.


CONCLUSIONS
We concluded that dental undergraduates and graduates had limited knowledge of the interactions of dental materials and prostheses with MRI. Dentists should be aware of interactions of MRI with the various dental materials and prostheses and possible image distortion to ensure the safety of the patient.

Imaging of status epilepticus: Making the invisible visible. A prospective study on 206 patients

Purpose. Many patients with metallic implants are aware of the potential dangers in an MRI environment. Some implants cause sensations perceptible by the patient during an MRI scan. We wanted to find out if patients having an implant abort MRI scans more often than patients without implants. Furthermore, we wanted to know if the number of implants of a patient has an influence on the number of aborted MRI scans. As we use 3T MRI scanners from different manufacturers we wanted to see the influence of the “ScanWise” software option where scan parameters are adapted automatically to the maximal allowed limits of implant values. This publication should help to reduce possible anxiety in patients with implants. Materials and Methods. From May 2017 until July 2018 the implant type of patients was recorded. We looked how often aborted MRI scans coincided with the occurrence of implants and the MRI scanner type used. Results. 4088 examinations were collected for the analysis. No influence of implants on aborted MRI scans was registered. No influence of the number of implants on aborted MRI scans was registered. No influence of the use of “ScanWise” software on aborted MRI scans was registered when different scanners were compared. Conclusion. “MR Safe” or “MR Conditional” labelled implants did not have an impact on the MRI examinations. “ScanWise” software does not affect how MRI examinations are tolerated.
