Diagnostic assessment of deep vein thrombosis and pulmonary embolism

Objective The aim of this study was to assess in clinical practice the accuracy of a referent d- dimer enzyme-linked immunosorbent assay for the exclusion of venous thromboembolic disease (VTED). Patients and Methods An observational prospective study took place in an emergency department; 205 consecutive outpatients suspected of having VTED were included. Blood samples were collected at admission for VIDAS DD measurement. Venous thromboembolic disease was confirmed by standard clinical imaging. All patients were followed up at 3 months. Results Venous thromboembolic disease was confirmed in 57 patients (28%). The sensitivity and negative predictive value of a DD assay lower than 500 ng/mL were 78% (95% confidence interval = 67%-87%) and 84% (95% confidence interval = 73%-90%), respectively. Twelve patients had a false-negative DD with one or more of the following: (a) symptoms reported for more than 15 days (n = 2), (b) prior anticoagulation (n = 3), (c) distal VTED (n = 5), or (d) high clinical probability (n = 3). Conclusion In our cohort of patients, DD was less accurate than previously reported, with an upper estimate of the sensitivity of only 87%.

Referent d-dimer enzyme-linked immunosorbent assay testing is of limited value in the exclusion of thromboembolic disease: result of a practical study in an ED.

Predicting Perioperative Venous Thromboembolism in Japanese Gynecological Patients

L'utilisation relativement recente du dosage des D-dimeres dans la demarche diagnostique de l'embolie pulmonaire est une aide precieuse en raison d'une excellente sensibilite. Cependant, les donnees concernant des patients atteints d'une embolie pulmonaire avec dosage des D-dimeres negatifs demeurent rares. Le but de ce travail etait d'etudier des patients avec embolie pulmonaire a D-dimeres negatifs et de les comparer a une population d'embolie pulmonaire a D-dimeres eleves. Methode : 150 patients consecutifs hospitalises pour une embolie pulmonaire ont ete inclus dans cette etude. Tous les patients avaient un dosage des D-dimeres (normal < 500 ng/mL) selon la technique ELISA. Les donnees cliniques et celles des differents examens complementaires ont ete analysees pour l'ensemble des patients en fonction du resultat des D-dimeres. Resultats: la sensibilite des D-dimeres etait de 96 % (6 patients avaient un dosage < 500 ng/mL). La presence d'une douleur thoracique etait statistiquement plus importante dans le groupe de patients a D-dimeres negatifs (p = 0.01). Pour ces patients, les embolies pulmonaires etaient toujours distales (p = 0.0003), l'index de Miller moyen etait significativement moins eleve par rapport aux patients a D-dimeres eleves (p = 0,04) et le rendement des examens ultrasonores (echocardiographie, echographie des membres inferieurs) etait plus faible (p < 0,0001). Conclusion: le dosage des D-dimeres par la methode ELISA peut etre pris en defaut en cas d'embolie pulmonaire distale et une des explications pourrait etre le faible processus thrombo-embolique. En cas de dosage negatif des D-dimeres, une suspicion clinique forte d'embolie pulmonaire doit conduire a la realisation d'autres investigations complementaires.
