Human rhinovirus (HRV) is a common cause of respiratory infections. HRV-related lower respiratory tract infections, including community-acquired pneumonia, are seldom seen in the clinical setting, and progression to acute respiratory distress syndrome (ARDS) is even rarer. We report on a case of a young immunosuppressed host who presented to the hospital for respiratory distress. She was diagnosed with HRV-related pneumonia, which rapidly progressed to ARDS based on clinical examination. After orotracheal intubation and mechanical ventilation with a low tidal volume strategy, she made a rapid recovery. This case highlights the importance of understanding that HRV may be an etiology of community-acquired pneumonia in immunosuppressed hosts and that ARDS may be a complication of this infection. Rapid recognition and clinical suspicion are important to the care of these patients, as ARDS has a high mortality rate. 

Acute Respiratory Distress Syndrome: A Rare Manifestation of Rhinovirus Infection

Acute portal vein thrombosis (APVT) is a rare condition characterized by recent thrombus formation within the main portal vein or its branches. APVT occurring in patients without underlying cirrhosis or malignancy represents an even rarer presentation, with an estimated prevalence of 0.7–3.7 per 100,000 individuals. However, it can lead to severe complications, including intestinal infarction and mortality. We report two cases presenting with abdominal pain without an apparent precipitating factor. Both patients were diagnosed with APVT based on contrast-enhanced computed tomography (CT) findings, clinical presentation, and laboratory parameters. Depending on the extent of portal vein occlusion, distinct therapeutic approaches were employed: one patient underwent interventional therapy combining transjugular mechanical thrombectomy/thrombolysis with transjugular intrahepatic portosystemic shunt (TIPS) placement, while the other received systemic pharmacological thrombolysis. Successful portal vein recanalization was achieved in both patients, who subsequently recovered and were discharged. These cases underscore that prompt diagnosis and management of APVT can avert adverse clinical outcomes. Contrast-enhanced CT demonstrates significant value in classifying APVT, assessing disease severity, evaluating treatment response, and identifying complications, thereby providing crucial evidence for clinical decision-making.

Portal vein thrombosis (PVT) and venous thromboembolism (VTE) both result from partial or complete occlusion of a blood vessel by a blood clot. The prognosis of PVT is generally good; however, PVT with VTE, including pulmonary embolism (PE), has a high mortality rate. We report here a case of PE after surgery for small intestinal necrosis caused by idiopathic PVT.A 69-year-old female attended our hospital with a chief complaint of upper abdominal discomfort, and was diagnosed with necrosis of the small intestine as a result of unexplained PVT. She underwent partial resection of the small intestine. On the second postoperative day, she suffered from respiratory distress and went into cardiopulmonary arrest. The patient recovered following cardiopulmonary resuscitation, but PE was detected. Extracorporeal veno-arterial cardiopulmonary resuscitation and anticoagulation therapy were initiated immediately and the thrombus was aspirated as much as possible. Two days later, extracorporeal veno-arterial cardiopulmonary resuscitation was withdrawn and anticoagulation therapy was continued. The patient subsequently recovered with no neurological damage and was discharged on day 26 after the above procedure.Idiopathic PVT is often associated with VTE, and a prompt diagnosis and intervention may result in a good prognosis.
